细节护理六步法在股骨颈骨折微创全髋关节置换术手术室护理中的应用价值

摘　　要：目的:探讨细节护理六步法在股骨颈骨折微创全髋关节置换术手术室护理中的应用价值。方法:选择我院2020年2月-2022年8月采用微创全髋关节置换术手术治疗的96例股骨颈骨折患者,将其均分为2组,对照组(48例)采取常规护理,观察组(48例)在对照组基础上采取细节护理六步法干预,观察与记录2组患者的预后情况。结果:(1)观察组的术中出血量、麻醉时间、手术时间、术前准备时间、术后住院时间等相比对照组均较短(P<0.05);(2)2组术后1个月并发症(压疮、便秘、静脉血栓、感染等)对比,观察组低于对照组(2.1%vs14.6%)(P<0.05);(3)2组术后1天、3天及7天的疼痛视觉模拟VAS评分比较,观察组下降幅度更大(P<0.05);(4)2组术后1周、1个月及3个月的髋关节Harris评分比较,观察组升高幅度更显著(P<0.05)。结论:细节护理六步法在股骨颈骨折微创全髋关节置换术手术室护理中的应用能减少应激创伤,缩短患者住院时间,缓解患者疼痛程度,促进患者髋关节功能的恢复。Objective:To investigate the application value of six-step detail nursing in the operating room nursing of minimally invasive total hip replacement for femoral neck fracture.Methods:From February 2020 to August 2022,96 patients with femoral neck fracture who received minimally invasive total hip replacement in Tianjin Fourth Central Hospital were selected and divided into two groups:48 patients in the control group received routine nursing,and 48 patients in the observation group received detailed nursing six-step intervention based on the control group.The prognosis of the two groups of patients was observed and recorded.Results:(1)Compared with the control group,the amount of intraoperative blood loss,anesthesia time,operation time,preoperative preparation time,postoperative hospital stay and so on were shorter in the observation group(P<0.05).(2)The complications(pressure ulcers,constipation,venous thrombosis,infection,etc.)in the observation group were lower than those in the control group one month after surgery(2.1%vs 14.6%,P<0.05).(3)The visual simulation(VAS)scores of postoperative day 1,3 and 7 between the two groups showed a greater decrease in the observation group(P<0.05).(4)The Harris hip score of the two groups at 1 week,1 month and3 months after surgery increased more significantly in the observation group(P<0.05).Conclusion:The application of six-step detail nursing in the operating room nursing of minimally invasive total hip replacement for femoral neck fracture can reduce stress and trauma,shorten the length of hospital stay,relieve the degree of pain,and promote the recovery of hip function.
