他汀类药物治疗出血性脑卒中研究进展

摘　　要：出血性脑卒中是脑卒中的一种,该病发生率比缺血性脑卒中低,但该病发生往往更为凶险,患者功能转归更差,病死率与致残率均更高。故而出血性脑卒中发生后,需引起重视,及时给予患者有效治疗。出血性脑卒中患者多患有脑动脉硬化、高血压,主要症状为意识障碍、呕吐、头痛、感觉障碍、偏瘫等,又称之为脑淤血。出血性脑卒中多以药物治疗,而他汀类药物为临床一线降血脂药,该药对脑卒中患者神经具有一定保护作用,而多项研究指出他汀类药物可较好改善缺血性脑卒中患者病情,对患者预后具有较为积极影响。而对于出血性脑卒中患者,他汀类药物治疗相关报道较少。目前,关于他汀类药物对出血性脑卒中患者预后影响仍存在较大争议,患者在住院期间是否可应用他汀类药物进行临床治疗至今仍然无法确定。故而本文对出血性脑卒中疾病病因及临床表现进行总结分析,综述了他汀类药物的分类与作用机制,以探究他汀类药物对出血性脑卒中患者的影响,为出血性脑卒中的临床治疗提供参考。Hemorrhagic stroke is a kind of stroke with a lower incidence than ischemic stroke,but it is often more dangerous,with worse functional outcomes of patients and higher mortality and disability rates.Therefore,attention should be paid to the occurrence of hemorrhagic stroke and effective treatment should be given to patients in time.Hemorrhagic stroke is also called cerebral congestion,patients are mostly with cerebral arteriosclerosis and hypertension,and the main symptoms are disturbance of consciousness,vomiting,headache,sensory disturbance,hemiplegia,etc.Hemorrhagic stroke is mostly treated with drugs,while statins are first-line clinical lipid-lowering agents,which have a certain protective effect on the nerves of patients with stroke.A number of studies have indicated that statins can better improve the condition of patients with ischemic stroke and have a positive effect on the prognosis of patients.However,there are few reports about statin therapy for patients with hemorrhagic stroke.At present,there is still a great controversy about the impact of statins on the prognosis of patients with hemorrhagic stroke,and it is still uncertain whether statins can be used for clinical treatment during hospitalization.Therefore,this paper summarizes and analyzes the etiology and clinical manifestations of action of hemorrhagic stroke,and reviews the classification and mechanism of statins to explore the effects of action of statins on patients with hemorrhagic stroke,in order to provide reference for the clinical treatment of hemorrhagic stroke.
