基于数据挖掘探讨中医药防治糖皮质激素不良反应用药规律

摘　　要：目的采用数据挖掘方法探讨中医药防治糖皮质激素不良反应的用药规律,为临床治疗提供参考。方法检索中国知网(CNKI)、万方数据知识服务平台(Wanfang Data)、维普中文期刊服务平台(VIP)、中国生物医学文献服务系统(SinoMed)建库至2022年11月有关中医药防治糖皮质激素不良反应的文献,剔除重复并筛选后建立方药数据库,借助Excel2019对涉及中医证型、药物频次及其性味归经进行统计分析,SPSS Modeler 18.0对涉及药物进行关联规则分析,SPSS Statistics 19.0对高频药物进行系统聚类分析。结果纳入文献152篇,涉及处方147首、中医证型136例、药物189味。频次前10位药物依次为茯苓、黄芪、生地黄、甘草、牡丹皮、知母、山药、白术、山萸肉、泽泻,药味以甘、苦、辛为主,药性以寒、温、平为主,药物归经以肾经、肝经、脾经为主,关联规则分析得到11组常用药对,聚类分析得到3首聚类方。结论中医药防治糖皮质激素不良反应多分阶段、分证型治疗,以补益肝肾、滋阴降火为主,兼顾痰湿瘀血等病理产物,标本同治。Objective To explore the medication law of TCM prevention and treatment for glucocorticoid side effects using data mining methods;To provide reference for clinical treatment.Methods The literature of TCM in the prevention and treatment of glucocorticoid side effects was retrieved from CNKI,Wanfang Data,VIP and SinoMed from the establishment of the databases to November,2022.A prescription database was established after literature review and screening.Excel 2019 was used to conduct statistical analysis on TCM syndrome types,medication frequency,and their properties,tastes,and meridians.SPSS Modeler 18.0 conducted association rule analysis on drugs involved,and SPSS Statistics 19.0 conducted systematic clustering analysis on drugs with higher frequency.Results Totally 152 articles were included,involving 147 prescriptions,136 cases of TCM syndrome types and 189 kind of Chinese materia medica.The top 10 kind of Chinese materia medica in frequency were Poria,Astragali Radix,Rehmannine Radix,Glycyrrhizae Radix et Rhizoma,Moutan Cortex,Anemarrhenae Rhizoma,Dioscoreae Rhizoma,Atractylodis Macrocephalae Rhizoma,Corni Fructus,and Alismatis Rhizoma.The main drug tastes were sweet,bitter and pungent.The main drug properties were cold,warm and mild.The meridians mainly involved the kidney,liver,spleen meridians.Association rule analysis obtained 11 groups of common drug pairs.Three cluster groups were obtained by clustering analysis.Conclusion TCM for the prevention and treatment of side effects of glucocorticoids is often divided into stages and syndrome types for treatment,with the main treatment of nourishing the liver and kidneys,nourishing yin and reducing fire,taking into account pathological products such as phlegm,dampness and blood stasis,and treating the symptoms and the root cause.
