摘 要:目的观察渴肾方对糖尿病肾病(DN)模型大鼠肾小管间质损伤的影响,探讨其潜在作用机制。方法40只SD大鼠随机分为正常组10只和造模组40只,造模组予高糖高脂饲料喂养8周联合1%链脲佐菌素30 mg/kg腹腔注射建立DN模型,将造模成功的30只大鼠随机分为模型组、渴肾方组和恩格列净组,每组10只,继续高糖高脂饲料喂养并以相应药物灌胃8周。采用血糖仪检测大鼠空腹血糖,全自动生化分析仪检测血清胆固醇含量,计算尿白蛋白/肌酐比率(UACR),PAS染色观察肾组织病理形态,Masson染色观察肾组织胶原纤维沉积,免疫组化染色检测肾组织α-平滑肌肌动蛋白(α-SMA)、CD68表达,Westernblot检测肾组织纤维连接蛋白(FN)、白细胞介素(IL)-6蛋白表达。结果与正常组比较,模型组大鼠空腹血糖、血清胆固醇、UACR明显升高(P<0.01),肾小管上皮细胞空泡样变,肾间质有大量炎性细胞浸润和胶原纤维沉积,肾组织α-SMA、CD68表达明显升高(P<0.01),FN、IL-6蛋白表达明显升高(P<0.01);与模型组比较,渴肾方组和恩格列净组大鼠空腹血糖、血清胆固醇、UACR明显降低(P<0.05),肾小管间质病理损伤明显改善,肾组织α-SMA、CD68表达明显降低(P<0.05),FN、IL-6蛋白表达明显降低(P<0.05)。结论渴肾方能改善DN大鼠肾小管间质损伤,其机制与抑制炎症反应、减轻肾纤维化有关。Objective To observe the effects of Keshen Prescription on tubulointerstitial injury in diabetic nephropathy(DN)model rats;To discuss its potential mechanism.Methods Totally 50 SD rats were randomly divided into normal group(10 rats)and modeling group(40 rats).A DN model was established by intraperitoneal injection of 1%streptozotocin 30 mg/kg and feeding with high sugar and high fat feed for 8 weeks in the modeling group,the 30 model rats were randomly divided into model group,Keshen Prescription group and empagliflozin group,with 10 rats in each group,and were fed with high sugar and high fat diet and received relevant medicine for gavage for 8 weeks.Glucose meter was used to detected fasting blood glucose,automated biochemical analyzer was used to detect serum cholesterol content,and urinary albumin/creatinine ratio(UACR)was calculated,the pathological morphology of renal tissue was observed using PAS staining,collagen fiber deposition in renal tissue was observed by Masson staining,immunohistochemical staining was used to detectα-smooth muscle actin(α-SMA),CD68 expression in renal tissue,Western blot was used to detect the expressions of fibronectin(FN)and interleukin-6(IL-6)proteins in renal tissue.Results Compared with the normal group,the fasting blood glucose,serum cholesterol content and UACR in the model group were significantly increased(P<0.01),with vacuolar degeneration of renal tubular epithelial cells,extensive infiltration of inflammatory cells and deposition of collagen fibers in renal interstitium,and the expressions ofα-SMA and CD68 in renal tissue significantly increased(P<0.01),the expressions of FN and IL-6 proteins significantly increased(P<0.01).Compared with the model group,the fasting blood glucose,serum cholesterol content and UACR in Keshen Prescription group and empagliflozin group were significantly decreased(P<0.05),with significant improvement in renal tubulointerstitial pathological damage,and the expression ofα-SMA and CD68 in renal tissue significantly decreased(P<0.05),whil
