锁定加压接骨板动力化联合自体骨移植术治疗股骨无菌性骨不连患者的效果

摘　　要：目的:观察锁定加压接骨板动力化联合自体骨移植术治疗股骨无菌性骨不连患者的效果。方法:选取2020年1月至2022年3月该院收治的90例股骨无菌性骨不连患者进行前瞻性研究,按随机数字表法将其分为对照组与观察组各45例。对照组采用自体骨移植术治疗,观察组采用锁定加压接骨板动力化联合自体骨移植术治疗,比较两组骨折愈合情况、成骨质量、手术前后关节活动度、治疗优良率和并发症发生率。结果:术后3、6、12个月,两组Lane-Sandhu X射线评分均高于术后1个月,且观察组高于对照组,差异有统计学意义(P<0.05);术后1个月,两组骨小梁面积、周长均大于术前,且观察组大于对照组,差异有统计学意义(P<0.05);术后6个月,两组膝关节、髋关节等关节活动度均高于术前,且观察组高于对照组,差异有统计学意义(P<0.05);观察组治疗优良率为91.11%(41/45),高于对照组的75.56%(34/45),差异有统计学意义(P<0.05);观察组并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论:锁定加压接骨板动力化联合自体骨移植术治疗股骨无菌性骨不连患者可提高Lane-Sandhu X射线评分、成骨质量、关节活动度和治疗优良率,降低并发症发生率,其效果优于单纯自体骨移植术治疗。Objective:To observe effects of dynamic locking compression plate combined with autologous bone grafting in treatment of patients with aseptic femoral nonunion.Methods:A prospective study was conducted on 90 patients with aseptic femoral nonunion admitted to this hospital from January 2020 to March 2022.They were divided into control group and observation group according to the random number table method,45 cases in each group.The control group was treated with autologous bone grafting,while the observation group was treated with dynamic locking compression plate combined with autologous bone grafting.The fracture healing,the bone quality,the range of motion before and after the surgery,the excellent and good rate of treatment,and the incidence of complications were compared between the two groups.Results:3,6 and 12 months after the surgery,the Lane-Sandhu X-ray scores of the two groups were higher than those 1 month after the surgery,those in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).1 month after the surgery,the area and perimeter of trabecular bone in the two groups were larger than those before the surgery,those in the observation group were larger than those in the control group,and the differences were statistically significant(P<0.05).6 months after the surgery,the range of motion levels of knee joint and hip joint in the two groups were higher than those before the surgery,those in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).The excellent and good rate of treatment in the observation group was 91.11%(41/45),which was higher than 75.56%(34/45)in the control group,and the difference was statistically significant(P<0.05).Further,the incidence of complications in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusions:The dynamic locking compression plate combined with autologo
