320排CT双血供灌注参数联合肿瘤标志物在术前肺癌纵隔淋巴结转移中的诊断效能

摘　　要：目的:分析320排CT双血供灌注(DI-CTP)参数联合肿瘤标志物在术前肺癌纵隔淋巴结转移中的诊断效能。方法:回顾性分析2013年1月至2022年1月该院收治的137例肺癌患者的临床资料,根据有无纵隔淋巴结转移分为转移组75例和未转移组62例;另选取同期50名健康体检者作为对照组。比较三组DI-CTP参数[肺动脉血流量(PAF)、灌注指数(PI)、支气管动脉血流量(BAF)]水平、肿瘤标志物[糖类抗原(CA)19-9、CA125、CA242、CA153、癌胚抗原(CEA)、甲胎蛋白(AFP)、铁蛋白(Ferritin)、绒毛膜促性腺激素β亚基(β-HCG)、神经元特异性烯醇化酶(NSE)]水平,并采用受试者工作特征曲线(ROC)分析320排CT DI-CTP参数、肿瘤标志物单项及联合检测诊断术前肺癌纵隔淋巴结转移的效能。结果:三组CA125、CA242、AFP、Ferritin、β-HCG、NSE等肿瘤标志物水平比较,差异均无统计学意义(P>0.05);转移组CA19-9、CA153、CEA水平均高于未转移组和对照组,且未转移组高于对照组,差异有统计学意义(P<0.05);三组PAF水平比较,差异均无统计学意义(P>0.05);转移组PI水平低于未转移组和对照组,且未转移组低于对照组,转移组BAF水平高于未转移组和对照组,且未转移组高于对照组,差异有统计学意义(P<0.05);ROC曲线分析结果显示,CA19-9、CA153、CEA、PI、BAF水平单项及联合检测诊断术前肺癌纵隔淋巴结转移的曲线下面积(AUC)分别为0.635、0.641、0.652、0.714、0.721、0.868,其中联合检测诊断的效能最高。结论:320排CT DI-CTP参数联合肿瘤标志物检测诊断术前肺癌纵隔淋巴结转移的效能高于两者单项检测诊断效能。Objective:To analyze diagnostic efficiency of 320-slice CT dual blood perfusion(DI-CTP)parameters combined with tumor markers in preoperative mediastinal lymph node metastasis of lung cancer.Methods:The clinical data of 137 patients with lung cancer admitted to this hospital from January 2013 to January 2022 were retrospectively analyzed.According to the presence or absence of mediastinal lymph node metastasis,they were divided into metastasis group(75 cases)and non-metastasis group(62 cases).Another 50 healthy subjects in the same period were selected as the control group.The levels of DI-CTP parameters[pulmonary artery blood flow(PAF),perfusion index(PI),bronchial artery blood flow(BAF)]and tumor markers[carbohydrate antigen(CA)19-9,CA125,CA242,CA153,carcinoembryonic antigen(CEA),alpha-fetoprotein(AFP),ferritin(Ferritin),chorionic gonadotropinβsubunit(β-HCG),neuron-specific enolase(NSE)]were compared among the three groups.The receiver operating curve(ROC)was used to analyze the efficiencies of single 320-slice CT DI-CTP parameter detection,single tumor marker detection and combined detection in the diagnosis of preoperative mediastinal lymph node metastasis of lung cancer.Results:There were no significant differences in the levels of CA125,CA242,AFP,Ferritin,β-HCG,NSE and other tumor markers among the three groups(P>0.05).The levels of CA19-9,CA153 and CEA in the metastasis group were higher than those in the non-metastasis group and the control group,those in the non-metastasis group were higher than those in the control group,and the differences were statistically significant(P<0.05).There were no significant differences in the level of PAF among the three groups(P>0.05).The level of PI in the metastasis group was lower than that in the non-metastasis group and the control group,and that in the non-metastasis group was lower than that in the control group;the BAF level in the metastasis group was higher than that in the non-metastasis group and the control group,and that in the non-metastasis group was hi
