卡格列净联合贝那普利治疗糖尿病肾病的临床效果

摘　　要：目的观察卡格列净联合贝那普利治疗糖尿病肾病的临床效果。方法选取2021年4月—2022年4月于新疆医科大学第五附属医院接受治疗的糖尿病肾病患者96例,采用随机数字表法分为观察组和对照组各48例。对照组仅接受贝那普利治疗,观察组在对照组的基础上接受卡格列净治疗,2组均连续治疗3个月。比较2组患者的临床疗效,治疗前后糖代谢指标[空腹血糖(FPG)、餐后2 h血糖(2 hPG)及糖化血红蛋白(HbA_(1c))]、脂代谢指标[总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)及三酰甘油(TG)]、肾功能指标[血肌酐(SCr)、血尿素氮(BUN)、尿蛋白排泄率(UAER)]及不良反应。结果观察组患者治疗后总有效率为95.83%,高于对照组的81.25%(χ^(2)=5.031,P=0.025)。治疗3个月后,2组患者的FPG、2 hPG水平及HbA_(1c)均低于治疗前,且观察组低于对照组(P<0.01);2组患者的TC、LDL-C、TG水平均低于治疗前,且观察组低于对照组(P<0.01);2组患者的SCr、BUN水平及UAER均低于治疗前,且观察组低于对照组(P<0.01)。2组患者治疗期间均未出现严重不良反应。结论卡格列净联合贝那普利治疗糖尿病肾病的临床效果肯定,可更有效地调节患者糖脂代谢,改善肾功能,安全性较高。Objective To observe the clinical effect of canagliflozin combined with benazepril in the treatment of diabetic nephropathy.Methods A total of 96 patients with diabetic nephropathy admitted to the Fifth Affiliated Hospital of Xinjiang Medical University from April 2021 to April 2022 were selected,the patients were divided into observation group and control group by random number table method,with 48 cases in each group,the control group was only treated with benazepril hydrochloride tablets,while the observation group was additionally treated with canagliflozin tablets on the basis of the control group,both groups were treated continuously for 3 months.The clinical efficacy,glucose metabolism indexes(FPG,2 hPG and HbA_(1c)),lipid metabolism indexes(TC,LDL-C and TG),and renal function indexes(SCr,BUN,UAER)before and after treatment,and adverse reactions were compared between two groups.Results The total effective rate of the observation group after treatment was 95.83%,which was higher than 81.25%of the control group(χ^(2)=5.031,P=0.025).After 3 months of treatment,the levels of FPG,2 hPG and HbA_(1c) in the two groups were lower than those before treatment,and the three items in the observation group were lower than those in the control group(P<0.01).The levels of TC,LDL-C and TG in the two groups were lower than those before treatment,and those in the observation group were lower than those in the control group(P<0.01).The levels of SCr,BUN and UAER in the two groups were lower than those before treatment,and the three items in the observation group were lower than those in the control group(P<0.01).No serious adverse reactions occurred in the two groups during treatment.Conclusion The clinical effect of captopril combined with benazepril in the treatment of diabetic nephropathy is positive,which can effectively regulate the glucose and lipid metabolism of patients,improve the renal function of patients,and has high safety.
