摘　　要：目的:观察尿激酶联合悬饮方对结核性包裹性胸膜炎胸腔积液患者临床体征及炎症指标的影响。方法:将78例结核性包裹性胸膜炎胸腔积液患者随机分为对照组(n=39)和观察组(n=39)。对照组术后给予尿激酶治疗,观察组在此基础上联合悬饮方,连续治疗2周。比较两组临床疗效、胸腔积液吸收时间、总引流量、胸膜厚度、中医证候积分、炎症指标、胸腔细胞因子及不良反应情况。结果:与对照组相比,观察组治疗总有效率高,胸腔积液吸收时间短,总引流量多,胸膜厚度、中医证候积分以及CRP、TNF-a、LDH等炎症指标低(均P<0.05);两组治疗期间未见严重不良反应。结论:尿激酶联合悬饮方可提高结核性包裹性胸膜炎胸腔积液患者治疗效果,促进积液排出,降低炎症指标及胸腔细胞因子水平,改善临床体征,且具有较高安全性。Objective:To observe the effects of urokinase combined with Xuanyin decoction on clinical signs and inflammatory indexes in patients with pleural effusion of tuberculous incarcerated pleurisy.Methods:Seventy-eight patients with pleural effusion of tuberculous encapsulated pleurisy were randomly divided into a control group(n=39)and an observation group(n=39).The control group was given urokinase treatment after surgery,and the observation group was given urokinase combined with Xuanyin decoction and the treatment was lasted for 2 weeks.The two groups were compared in terms of clinical efficacy,pleural effusion absorption time,total drainage volume,pleural thickness,Chinese medicine syndrome points,inflammation indexes,pleural cytokines and adverse reactions.Results:Compared with the control group,the observation group had a high total efficacy rate,short pleural effusion absorption time,high total drainage volume,low pleural thickness,low Chinese medicine evidence points and inflammatory indexes such as CRP,TNF-a,and LDH(all P<0.05).No serious adverse reactions were found during the treatment.Conclusion:Urokinase combined with Xuanyin decoction can improve the therapeutic effect of the patients,promote the effusion discharge,reduce the inflammatory indexes and the level of pleural cytokines and improve the clinical signs,and it has a high safety.
