2017~2021年某院大肠埃希菌对常用抗菌药物耐药性分析

摘　　要：目的 了解某院近5年来大肠埃希菌的检出情况及对临床主要使用抗菌药物的耐药性,为临床经验用药提供依据。方法 回顾性分析医院2017年1月~2021年12月检出1 774份大肠埃希菌的标本分布及其对医院主要使用抗菌药物的耐药情况。结果 5年来医院大肠埃希菌检出率26.32%(1 774/6 741),主要来自尿标本,占50.73%。药敏分析表明,大肠埃希菌对头孢噻吩、头孢呋辛、左氧氟沙星、环丙沙星、哌拉西林耐药率> 50%,对亚胺培南、美罗培南、阿米卡星等耐药率<10%。结论 医院大肠埃希菌检出率呈逐年上升趋势,以泌尿道感染为主,建议关注患者感染危险因素。治疗时可选用阿米卡星、哌拉西林他唑巴坦作为一线治疗药物,不宜选择庆大霉素、第1、2代头孢菌素和环丙沙星、左氧氟沙星作为单药治疗的首选药物。Objective The detection status of Escherichia coli in a certain hospital in the past 5 years and the resistance to the main antibiotics used in clinical practice provide a basis for clinical experience in drug use.Methods Retrospective analysis of the distribution of 1774 Escherichia coli specimens detected in hospitals from January 2017 to December 2021 and their resistance to the main antibiotics used in the hospital.Results In the past 5 years,the hospital detection rate of Escherichia coli was 26.32%(1774/6741),mainly from urine samples,accounting for 50.73%.The drug sensitivity analysis showed that the drug resistance rate of Escherichia coli to cefothiophene,cefuroxime,levofloxacin,ciprofloxacin,piperacillin was more than 50%,and the drug resistance rate to imipenem,meropenem,amikacin was less than 10%.Conclusion The detection rate of Escherichia coli in hospitals is increasing year by year,with urinary tract infections being the main cause.It is recommended to pay attention to the risk factors of infection in patients.Amikacin and piperacillin tazobactam can be selected as first-line treatment drugs during treatment,and gentamicin,first and second generation cephalosporins,ciprofloxacin,and levofloxacin should not be chosen as the preferred drugs for monotherapy.
