六君子汤联合穴位贴敷治疗慢性阻塞性肺疾病稳定期伴便秘的临床研究

摘　　要：目的 评价六君子汤联合穴位贴敷治疗慢性阻塞性肺疾病稳定期伴便秘临床效果。方法 将2019年2月~12月在某社区卫生服务中心就诊的50例慢性阻塞性肺疾病稳定期伴便秘患者按照来社区卫生服务中心就诊时间顺序进行编号,单号为观察组,双号为对照组,各25例。对照组采用西医支气管舒张剂、祛痰剂、吸入糖皮质激素和六君子汤药方治疗方案,观察组在对照组的基础上联合中药穴位贴敷治疗。比较两组患者治疗前和治疗后便秘主要症状评分、便秘患者生活质量自评量表(PAC-QOL)评分和临床治疗效果。结果 两组患者经治疗后粪便性状、排便费力、下坠和不尽胀感、腹胀、排便频率评分比治疗前均有下降(P <0.01);观察组患者便秘主要症状评分均比对照组要低(P <0.01);两组患者治疗后PAC-QOL评分和总分均比治疗前低(P <0.01),观察组中4个因子的评分和总分均比对照组低(P <0.05)。治疗后,观察组患者临床治疗效果明显优于对照组(Z=2.105,P <0.05)。结论 六君子汤联合穴位贴敷能有效减轻慢性阻塞性肺疾病稳定期功能性便秘患者症状,增加排便次数,提高临床有效率和患者的生活质量。Objective To evaluate the clinical effect of Liujunzi Decoction combined with acupoint application on chronic obstructive pulmonary disease(COPD)with constipation in stable stage.Methods In this study,50 cases of COPD patients with constipation in stable phase who were treated in our Community Healthcare Center from February to December 2019 were numbered according to the time sequence of their visit to our Community Healthcare Center.The single number is the observation group,and the double number is the control group,with 25 cases each.The control group was treated with Western bronchodilator,expectorant,inhalation of glucocorticoid and Liujunzi Decoction.The observation group was treated with point application of traditional Chinese medicine on the basis of the control group.The main symptom score,PAC-QOL score and clinical effect of constipation before and after treatment were compared between the two groups.Results After treatment,the score of stool character,defecation effort,falling and distension,abdominal distension and frequency of defecation decreased in both groups(P<0.01);the score of major symptoms of constipation in the observation group was lower than that in the control group(P<0.01);the score of PAC-QOL and total score in both groups were lower than that before treatment(P<0.01),and the scores of four factors in the observation group were lower than those in the control group(P<0.05).After treatment,the clinical effect of the observation group was significantly better than that of the control group(Z=2.105,P<0.05).Conclusion Liujunzi Decoction combined with acupoint application can effectively relieve the symptoms of chronic obstructive pulmonary disease patients with stable functional constipation,increase the frequency of defecation,improve the clinical efficiency,improve the quality of life of patients.
