半夏白术天麻汤联合针灸治疗皮层下动脉硬化性脑病的效果

摘　　要：目的探讨半夏白术天麻汤联合针灸治疗在皮层下动脉硬化性脑病患者中的应用效果。方法选取天津市中医药研究院附属医院脑三科某院2021年3月~2023年2月80例皮层下动脉硬化性脑病患者,按照随机数字法分为对照组(n=40)和观察组(n=40),其中对照组采用常规西医治疗;观察组采用半夏白术天麻汤联合针灸治疗。评估治疗临床疗效、中医证候积分、血清生化指标、智力评分、血液流变学。结果观察组的总有效率高于对照组(P<0.05);治疗14 d、60 d后观察组的中医证候积分均低于对照组(P<0.05);治疗后观察组的同型半胱氨酸(Hcy)、总胆固醇(TC)水平及血液流变学指标均低于对照组(P<0.05),高密度脂蛋白(HDL)水平高于对照组(P<0.05);治疗14 d、60 d后观察组的长谷川智力量表(HDS)评分均高于对照组(P<0.05)。结论皮层下动脉硬化性脑病患者采用半夏白术天麻汤联合针灸治疗效果显著,改善血液流变学和血清生化指标,缓解患者的症状,改善患者的智力。Objective To investigate the effect of Banxia Baizhu Tianma Decoction combined with acupuncture in the treatment of patients with subcortical arteriosclerotic encephalopathy.Methods 80 patients with subcortical arteriosclerotic encephalopathy in our hospital from March 2021 to February 2023 were selected and divided into the control group(n=40)and the observation group(n=40)according to random number method.The control group was treated with conventional Western medicine.The observation group was treated with Banxia Baizhu Tianma Decoction combined with acupuncture.The clinical effect,TCM syndrome score,serum biochemical index,intelligence score and hemorheology were evaluated.Results The total effective rate of the observation group was higher than that of the control group(P<0.05).After 14 and 60 days of treatment,the TCM syndrome scores in the observation group were lower than those in the control group(P<0.05).After treatment,the levels of Hcy,TC and hemorheology in the observation group were lower than those in the control group(P<0.05),and the level of HDL was higher than that in the control group(P<0.05).The HDS scores of the observation group were higher than those of the control group after 14 and 60 days of treatment(P<0.05).Conclusion Banxia Baizhu Tianma Decoction combined with acupuncture and moxibustion can improve the hemorheology and serum biochemical indexes,relieve the symptoms and improve the intelligence of the patients with subcortical arteriosclerotic encephalopathy.
