摘　　要：目的分析重组人脑利钠肽(rhBNP)联合沙库巴曲缬沙坦钠对冠心病心力衰竭患者的疗效。方法选取2020年10月至2022年10月该院收治的冠心病心力衰竭患者240例,按不同治疗方案分为A组和R组,每组120例。A组在常规治疗基础上加用沙库巴曲缬沙坦钠片口服治疗,R组在常规治疗基础上采用rhBNP联合沙库巴曲缬沙坦钠片治疗,疗程均为4周。记录2组患者治疗1周后一般临床指标(包括收缩压、心率和平均每小时尿量)、治疗4周后心功能相关指标(包括左心室射血分数、左心室舒张末期内径和每搏输出量),以及血清线粒体偶联因子6、心锚重复蛋白、细胞黏附分子-1、醛固酮水平等。结果R组患者治疗1周后心率、收缩压均明显低于A组,平均每小时尿量明显高于A组,差异均有统计学意义(P<0.05);治疗4周后左心室射血分数、每搏输出量均明显高于A组,左心室舒张末期内径,以及血清线粒体偶联因子6、心锚重复蛋白、细胞黏附分子-1、醛固酮水平均明显低于A组,差异均有统计学意义(P<0.05)。结论rhBNP联合沙库巴曲缬沙坦钠可快速缓解冠心病心力衰竭患者临床症状,有效改善心功能,效果显著,值得借鉴。Objective To analyze the therapeutic effects of recombinant human brain natriuretic peptide(rhBNP)combined with sacubitril valsartan sodium in the treatment of heart failure and coronary heart disease.Methods A total of 240 patients admitted to the hospital from October 2020 to October 2022 were selected and divided into group A and group R according to different treatment schemes,with 120 patients in each group.Group A was treated with oral sacubitril valsartan sodium tablets on the basis of routine treatment,and group R was treated with rhBNP combined with sacubitril valsartan sodium tablets on the basis of routine treatment.The course of treatment was four weeks.The general clinical indexes(including systolic blood pressure,heart rate and average hourly urine volume)after one week of treatment,cardiac function related indexes(including left ventricular ejection fraction,left ventricular end diastolic diameter and stroke output)mitochondrial coupling factor 6(CF6),cardiac anchor repeat protein(CARP),cell adhesion molecule-1(ICAM-1)and aldosterone(ALD)were recorded in the two groups after treatment for one week.Results After one week of treatment,the heart rate and systolic blood pressure in group R were significantly lower than those in group A,and the average urine volume per hour was significantly higher than that in group A,and the differences were statistically significant(P<0.05).After four weeks of treatment,LVEF and SV in group R were significantly higher than those in group A,and LVED was significantly lower than that in group A;the levels of serum CF6,CARP,ICAM-1 and ALD in group R were significantly lower than those in group A,and the differences were statistically significant(P<0.05).Conclusion The rhBNP combined with sacubitril valsartan sodium can rapidly relieve the clinical symptoms of patients with heart failure and coronary heart disease,and effectively improve the cardiac function.The effect is significant,which is worthy of reference.
