摘  要:目的 探讨合成MRI技术对鉴别及诊断乳腺良恶性病变的价值。方法 回顾性分析92例乳腺疾病患者的临床、病理及合成MRI影像资料,通过设备自带的MAGiC后处理软件定量测量影像学参数值:T1值、T2值、质子密度(PD值),比较良恶性乳腺病变患者的病理、临床及影像参数的差异性,通过ROC曲线比较定量参数值对乳腺良恶性病变鉴别及诊断效能。结果 良性病变与恶性病变的T2值分别为118.00(106.50,129.50) ms、90.00(82.00,100.00) ms, PD值分别为(101.7±13.13) pu(pu为单位体素内氢质子密度)、(75.58±13.91) pu,差异具有统计学意义(P<0.05);以T2值=97.0 ms、PD值=79.6 pu为界值,鉴别良恶性乳腺病变的ROC曲线下面积分别为0.873、0.901,特异度分别为95.56%、100.00%,灵敏度分别为72.34%、70.21%。结论 合成MRI技术定量参数中的T2、质子密度(PD值)在鉴别良恶性乳腺病变的诊断中具有一定的价值。Objective To investigate the diagnostic value of synthetic MRI in distinguishing benign and malignant breast lesions.Methods The clinical,pathological and synthetic MRI image data of 92 patients with breast diseases were retrospectively analyzed,and the imaging parameters were quantitative measureed by MAGiC post-processing software:T1,T2,proton density(PD value),compare the differences of clinical,pathological and imaging parameters of the patients with benign and malignant breast lesions,using the ROC curve to compare the quantitative parameter value in differential and diagnostic efficiency of benign and malignant breast lesions.Results The T2 values of benign and malignant lesions were 118.00(106.50,129.50)ms and 90.00(82.00,100.00)ms,The PD values were(101.7±13.13)pu(hydrogen proton density within unit voxel)and(75.58±13.91)pu,respectively,and the difference was statistically significant(P<0.05).With T2 value=97.0 ms and PD value=79.6 pu as the cut-off value,the area under ROC curve for distinguishing benign and malignant breast lesions was 0.873 and 0.901,the specificitywas 95.56%and 100.00%,and the sensitivity was 72.34%and 70.21%respectively.Conclusion T2 and proton density(PD value)in the quantitative parameters of synthetic MRI technology have certain value in the diagnosis of benign and malignant breast lesions.
