磁共振成像影像组学特征与乳腺癌分子分型的相关性研究

摘　　要：目的 探讨磁共振成像(Magnetic resonance imaging, MRI)影像组学特征与乳腺癌分子分型的相关性。方法 选取2020年11月-2022年11月扬州大学附属泰州第二人民医院乳腺癌患者143例作为研究对象,所有患者均在术后切除乳腺癌病灶标本进行分子分型鉴定、血管内皮生长因子(Vascular endothelial growth factor, VEGF)表达检测,并实施MRI影像组学相关检查。统计所有乳腺癌患者分子分型、不同分子分型乳腺癌患者MRI影像组学相关参数及VEGF表达,并分析不同分子分型乳腺癌患者MRI影像组学相关参数与VEGF表达的相关性。结果 143例乳腺癌患者中Luminal A型49例(34.27%)、Luminal B型45例(31.47%)、Her-2过表达型21例(14.69%)、三阴型28例(19.58%)。乳腺癌不同分子分型者血管外细胞间隙容积比(Extracellular extravascular volume fraction, Ve)、速率常数(Rate constant, Kep)、容积转移常数(The influx volume transfer constant, Ktrans)、表观弥散系数(Apparent diffusion coefficient, ADC)及VEGF表达水平比较,差异均有统计学意义(P<0.05);且Luminal A型患者Ve、ADC值高于人表皮生长因子受体-2(Human Epidermal growth factor Receptor 2,Her-2)过表达型、三阴型患者,Kep、Ktrans、VEGF低于Her-2过表达型、三阴型患者,差异均有统计学意义(P<0.05);Luminal B型患者Ve、ADC值高于Her-2过表达型、三阴型患者,Kep、Ktrans、VEGF低于Her-2过表达型、三阴型患者,差异均有统计学意义(P<0.05);Her-2过表达型患者Ve高于三阴型患者,VEGF低于三阴型患者(P<0.05),其余指标两两比较差异无统计学意义(P>0.05)。乳腺癌4种分子分型者,Ve、ADC值与VEGF均呈负相关(r_(1)=-0.389、-0.375、-0.344、-0.408,r_(2)=-0.362、-0.385、-0.344、-0.426,P<0.05),Kep、Ktrans与VEGF均呈正相关(r_(1)=0.371、0.376、0.418、0.424,r_(2)=0.338、0.387、0.451、0.456,P<0.05)。结论 乳腺癌不同分子分型者MRI影像组学相关参数与VEGF表达差异具有相关性,临床�Objective To investigate the correlation between the imaging features of magnetic resonance imaging(MRI)and the molecular typing of breast cancer.Methods From November 2020 to November 2022,143 breast cancer patients in the hospital were selected as the research objects.All patients were ex-cised from breast cancer after the operation.Molecular typing and vascular endothelial growth factor(VEGF)expression test were performed,and MRI imaging correlation examination was performed.The molecular typing of breast cancer patients,the MRI imaging correlation parameters and VEGF expression of breast cancer patients with different molecular subtypes were analyzed statistically,and the correlation between MRI imaging parameters and VEGF expression were analyzed.Results The 143 patients with breast cancer in this group included:Luminal A 49 cases(34.27%),Luminal B 45 cases(31.47%),Her-2 overexpression 21 cases(14.69%)and triple negative 28 cases(19.58%).Extracellular extravascular volume fraction(Ve)and Rate constant(VE)of different molecular types of breast cancer Kep,the influx volume transfer constant(Ktrans),Apparent diffusion coefficient(ADC value)and VEGF expression were compared.The difference was statistically significant(P<0.05).And the Ve and ADC values of Luminal A patients were higher than those of human epidermal growth factor Receptor 2(Her-2)overex-pression and triple-negative patients.Kep,Ktrans and VEGF were lower than those of Her-2 overexpres-sion type and triple-negative type,the difference was statistically significant(P<0.05).Ve and ADC val-ues in Luminal B patients were higher than those in Her-2 overexpression type and triple-negative type,and Kep,Ktrans and VEGF were lower than those in Her-2 overexpression type and triple-negative type,with statistical significance(P<0.05).Her-2 overexpression type patients with Ve was higher than tri-ple-negative type patients,VEGF was lower than triple-negative type patients(P<0.05),and there was no statistically significant difference in other indicators(P>0.05)
