针刺治疗对预防腹腔镜术后恶心呕吐有效性的Meta分析

摘　　要：目的系统评价针刺治疗腹腔镜术后恶心呕吐(PONV)的疗效。方法检索中国知网(CNKI)、中国生物医学文献(CBM)、万方(Wanfang)和PubMed、Cochrane library、Web of science、Embase数据库建库以来至2022年9月1日关于针刺治疗腹腔镜术后恶心呕吐的随机对照研究(RCT)。由2名研究人员独立完成文献筛选、数据提取和文献质量评价,并采用RevMan5.4软件进行统计分析。结果共纳入16篇RCT,患者共1900例,其中针刺组1020例,对照组880例。Meta分析结果显示:针刺组24 h内PONV发生率较对照组低,差异有统计学意义(RR=0.52,95%CI 0.43~0.62,P<0.05);两组患者24 h内PONV评分(SMD=-0.24,95%CI-0.54~0.06,P>0.05)、术后24 h止吐药物需求率(RR=0.62,95%CI 0.33~1.15,P>0.05)和术后头晕发生率(RR=0.56,95%CI 0.25~1.24,P>0.05)比较,差异均无统计学意义。结论针刺疗法能降低腹腔镜PONV的发生率,有效恢复术后胃肠功能。Objective A systematic review of the efficacy of acupuncture in the treatment of postoperative nausea and vomiting(PONV)after laparoscopic surgery.Methods CNKI,CBM,Wangfang,PubMed,Cochrane library,Web of science,Embasedatabases were systematically searched to collect randomized controlled trials(RCTs)on acupuncture in the treatment of nausea and vomiting after laparoscopy until September 1,2022.2 researchers independently completed literature screening,data extraction and literature quality evaluation,and used RevMan5.4 software for statistical analysis.Results A total of 16 RCTs were included,involving 1900 patients,including 1020 in the acupuncture group and 880 in the control group.Meta-analysis results showed that the incidence of PONV within 24 hours in the acupuncture group was lower than that in the control group,and the difference was statistically significant(RR=0.52,95%CI 0.43-0.62,P<0.05);PONV scores within 24 hours of 2 groups of the patients(SMD=-0.24,95%CI-0.54-0.06,P>0.05),the demand rate of antiemetic drugs 24 hours after the operation(RR=0.62,95%CI 0.33-1.15,P>0.05),the incidence of postoperative dizziness(RR=0.56,95%CI 0.25-1.24,P>0.05),there was no statistical significant.Conclusion Acupuncture therapy can reduce the incidence of laparoscopic PONV and effectively restore postoperative gastrointestinal function.
