血管超声检查在髋部骨折患者术前排除下肢深静脉血栓中的价值

摘　　要：目的探讨血管超声检查在髋部骨折患者手术前排除下肢深静脉血栓(deep vein thrombosis,DVT)中的价值。方法回顾性分析荆州市第三人民医院2016-01-2022-04月收治的1000例髋部骨折患者,其中粗隆间骨折498例(男261例,女237例)、股骨颈骨折502例(男201例,女301例)。术前常规行双侧下肢静脉多普勒超声检查,依据多普勒超声检查结果判断其有无下肢DVT形成。结果术前粗隆间骨折患者DVT阳性率为21.89%(109/498),股骨颈骨折患者DVT阳性率18.92%(95/502)。股骨粗隆间骨折术前实施下腔静脉造影滤器植入术16例,股骨颈骨折术前实施下腔静脉造影滤器植入术14例,总植入滤器比例3.0%。结论髋部骨折患者易发生DVT,术前常规行多普勒超声检查可及时准确检出DVT部位,对术前采取合理的干预措施、防止血栓脱落导致肺栓塞(pulmonary embolism,PE)具有重要意义。Objective To explore the diagnostic value of vascular ultrasound in preoperative exclusion of lower limb deep vein thrombosis(DVT)in patients with hip fractures.Methods 1000 cases of patients with hip fractures admitted to Jingzhou Third People's Hospital from January 2016 to April 2022 were retrospective analysed,including 498 cases of intertrochanteric fractures(261 males and 237 females)and 502 cases of femoral neck fractures(201 males and 301 females).Routine examination of bilateral lower limb vein by Doppler ultrasound before surgery were performd,and whether there is lower limb DVT formation was determined based on the results of Doppler ultrasound examination.Results The positive rate of DVT in patients with intertrochanteric fracture was 21.89%(109/498),and the positive rate of DVT in patients with femoral neck fracture was 18.92%(95/502).16 cases of patients with femoral intertrochanteric fracture underwent preoperative implantation of inferior vena cava imaging filters,and 14 cases of patients with femoral neck fracture underwent preoperative implantation of inferior vena cava imaging filters,with a total implantation rate of 3.0%.Conclusion Hip fracture patients are prone to DVT,and routine Doppler ultrasound examination before surgery can timely and accurately detect the location of DVT.It is of great significance to take reasonable intervention measures before surgery to prevent pulmonary embolism(PE)caused by thrombus detachment.
