芪参还五胶囊配合序贯针刺法对脑出血术后康复期患者意识恢复和肢体功能的影响

摘　　要：目的观察芪参还五胶囊配合“皮层-面部-四肢”序贯针刺法对脑出血术后康复期患者意识恢复和肢体功能的影响。方法选择符合纳入标准的脑出血术后康复期患者126例,按照随机数字表法分为对照组和观察组。对照组给予相应的基础治疗,加用甘露醇注射液和依达拉奉注射液;观察组在对照组的基础上给予芪参还五胶囊配合“皮层-面部-四肢”序贯针刺法(皮层针刺点、舌咽部针刺法和体表针刺法)。两组均治疗2周,观察治疗前后两组患者简式Fugl-Meyer运动功能评分(FMA)、美国国立卫生院神经功能缺损评分(NIHSS)和格拉斯哥预后评分(GOS)的变化情况。结果治疗过程中,观察组因不耐受针灸治疗退出2例,对照组病情加重1例,最终对照组纳入观察病例62例,观察组纳入观察病例61例。两组治疗前FMA、NIHSS和GOS评分无差异(P>0.05)。治疗后两组上肢FMA、下肢FMA和NIHSS评分均升高,GOS评分降低(P<0.01)。治疗后观察组FMA、NIHSS和GOS评分的改善情况优于对照组(P<0.01)。结论芪参还五胶囊配合“皮层-面部-四肢”序贯针刺法可以明显促进脑出血术后康复期患者意识恢复,并改善肢体功能,值得临床推广。Objective To observe the effect of Qishen Huanwu capsule combined with"cortex face limbs"sequential acupuncture on consciousness recovery and limb function of patients with intracerebral hemorrhage.Methods 126 patients with postoperative rehabilitation of intracerebral hemorrhage who met the inclusion criteria were randomly divided into control group and observation group.The control group was given corresponding basic treatment,plus mannitol injection and edaravone injection;On the basis of the control group,the observation group was given Qishen Huanwu capsule combined with"cortex face limbs"sequential acupuncture(cortex acupuncture point,glossopharyngeal acupuncture and body surface acupuncture).Both groups were treated for 2 weeks.The changes of simple Fugl Meyer motor function score(FMA),National Institutes of health neurological deficit score(NIHSS)and Glasgow prognosis score(GOS)were observed before and after treatment.Results During the treatment,2 cases in the observation group withdrew from acupuncture treatment due to intolerance,1 case in the control group became worse,so finally 62 cases were included in the control group and 61 cases were included in the observation group.There was no significant difference in the scores of FMA,NIHSS and GOS between the two groups before treatment(P>0.05).After treatment,the scores of upper limb FMA,lower limb FMA and NIHSS increased and GOS decreased in the two groups,which was statistically significant compared with that before treatment(P<0.01).After treatment,the improvement of FMA,NIHSS and GOS scores in the observation group was better than that in the control group(P<0.01).Conclusion Qishen Huanwu capsule combined with"cortex face limbs"sequential acupuncture can significantly promote the recovery of consciousness and improve limb function for patients with intracerebral hemorrhage in the postoperative rehabilitation period,which is worthy of clinical promotion.
