全内镜前路经椎体联合后路经椎板间隙治疗双节段颈椎间盘突出1例报告

摘　　要：目的初步探讨联合入路全内镜技术治疗混合型双节段颈椎间盘突出症的可行性。方法对本院收治的1例混合型双节段颈椎间盘突出的患者全内镜前路经椎体与后路经椎板间隙入路进行治疗,并使用相应临床评分与影像学手段对手术效果进行监测。结果患者各项临床症状都得到了缓解,VAS评分显示颈部及三角肌区疼痛明显缓解,JOA评分显示颈部功能得到明显恢复。MRI显示突出椎间盘被有效摘除,CT显示椎体内镜通道获得了骨性愈合,X线显示颈椎生理曲度存在,无颈椎不稳。结论联合入路全内镜技术治疗混合型双节段颈椎间盘突出症创伤小、恢复快,初步展示了其可行性。Objective To preliminarily explore the feasibility of a combined approach full endoscopic technique for the treatment of mixed two-segment cervical disc herniation.Methods A patient admitted to our hospital with a mixed two-segment cervical disc herniation was treated with a combined anterior transcorporeal full-endoscopic approach and a posterior translaminar approach full endoscopic technique,and the results of the procedure were monitored using appropriate clinical scores and imaging methods.Results All clinical symptoms were relieved.The VAS score showed significant relief of pain in the neck and deltoid region;the JOA score showed significant recovery of neck function.the MRI showed that the herniated disc was effectively removed;the CT showed that bony healing was obtained in the endoscopic channel of the vertebral body,and the X-ray showed the cervical physiological curvature was present and there was no cervical instability.Conclusion The combined approach full endoscopic technique for the treatment of mixed two-segment cervical disc herniation is less invasive and has rapid recovery,and demonstrates its feasibility initially.
