基于区域卫生信息平台的上海市徐汇区签约居民人群结构分析

摘　　要：目的:收集、汇总分析2016—2021年上海市徐汇区签约居民的一般资料及就诊情况等,为精准推进家庭医生签约服务提供依据。方法:选取于2016年1月1日—2021年12月31日与上海市徐汇区13家社区卫生服务中心家庭医生签约的社区居民作为调查对象,通过徐汇区区域卫生信息平台收集、汇总分析其性别、年龄、高血压与糖尿病患病情况、签约医疗机构、就诊情况等信息。结果:2021年徐汇区签约居民平均就诊次数为14.95次,在签约社区卫生服务中心的平均就诊次数为6.50次,≥60岁签约居民的平均就诊次数最高。2021年徐汇区签约居民中,20.36%没有当年就诊记录,低活跃度、中活跃度的签约居民比例分别为37.89%、35.08%;在签约社区卫生服务中心的就诊率为43.47%,在签约社区卫生服务中心的就诊依从性为84.10%。结论:应通过扩大家庭医生签约服务覆盖面、拓展签约服务内涵、开展精细化和个性化健康服务等措施,进一步开展家庭医生签约服务工作。Objective:To collect,summarize and analyze the general information and medical treatment status of contracted residents in Xuhui District,Shanghai from 2016 to 2021 to provide a basis for precise promotion of family doctor contract services.Methods:The community residents contracted with family doctors in 13 community health service centers in Xuhui District of Shanghai from January 1,2016 to December 31,2021 were selected as the survey objects,and their gender,age,prevalence of hypertension and diabetes,contracted medical institutions,medical treatment and other information were collected,summarized and analyzed through the regional health information platform in Xuhui District.Results:In 2021,the average number of visits for contracted residents in Xuhui District was 14.95,and the average number of visits at contracted community health service centers was 6.50,and the average number of visits for contracted residents aged≥60 was the highest.In 2021,20.36%of contracted residents in Xuhui District did not have a medical record for that year,while the proportion of low activity and medium activity contracted residents was 37.89%and 35.08%,respectively;the visit rate at the contracted community health service centers was 43.47%,and the visit compliance at the contracted community health service centers was 84.10%.Conclusion:Measures such as expanding the coverage of family doctor contracted services,expanding the connotation of contracted services,and carrying out refined and personalized health services should be taken to furhter carry out the work of family doctor contracted services.
