摘 要:目的:探讨消黄去疸汤改善胆汁淤积性肝损伤的疗效。方法:将28只SD大鼠按随机数字表法分为空白组(NOM)、模型组(MOD)、熊去氧胆酸组(UDCA)和消黄去疸汤组(XHQD),每组7只,各组大鼠每天给予相应药物灌胃1次,连续7 d。至第5天时,除NOM组外,其余各组大鼠予以α-萘异硫氰酸酯(ANIT)灌胃诱导胆汁淤积性肝损伤模型。监测大鼠90 min胆汁流量,计算各组大鼠肝脏指数,HE染色观察大鼠肝脏组织病理变化,透射电镜观察肝细胞超微结构,全自动生化仪检测血清中碱性磷酸酶(ALP)、丙氨酸氨基转移酶(ALT)、天门冬氨酸氨基转移酶(AST)、γ-谷氨酰转肽酶(γ-GT)、总胆红素(TBil)、直接胆红素(DBil)与总胆汁酸(TBA)含量,Elisa法检测大鼠肝组织中超氧化物歧化酶(SOD)、还原性谷胱甘肽(GSH)与丙二醛(MDA)的含量。采用qRT-PCR法测定大鼠肝组织中TNF-α、IL-1β以及IL-6 mRNA的表达。结果:与NOM组比较,MOD组大鼠在各个时间段胆汁流量降低,AST、ALT、ALP、γ-GT含量增加,肝脏指数升高,DBil、TBil、TBA水平升高,SOD活性及GSH、MDA水平降低,TNF-α、IL-1β以及IL-6 mRNA水平升高,差异均具有统计学意义(P<0.01)。与MOD组大鼠比较,UDCA组大鼠15、30、75、90 min时间段胆汁流量升高,XHQD组大鼠各个时间段胆汁流量均升高,UDCA与XHQD组大鼠AST、ALT、ALP、γ-GT含量减少,肝脏指数降低,DBil、TBil、TBA水平降低,SOD活性及GSH水平降低、MDA水平升高,SOD活性及GSH水平降低、MDA水平升高,TNF-α、IL-1β以及IL-6 mRNA水平降低,差异均具有统计学意义(P<0.05,P<0.01)。结论:消黄去疸汤对胆汁淤积型肝损伤大鼠具有明显的保护作用,其作用机制可能是调节氧化应激及炎症反应。Objective:To explore the therapeutic effect of Xiaohuang Qudan decoction on cholestatic liver injury.Methods:Twenty-eight SD rats were divided into blank group(NOM),model group(MOD),ursodeoxycholic acid group(UDCA)and Xiaohuang Deicterice Decoction group(XHQD)according to random number table,with 7 rats in each group.The rats in each group were given the corresponding drug once a day by intragastric administration for 7 consecutive days.On the 5 day,except for the NOM group,other groups were gavaged withα-naphthylisothiocyanate(ANIT)to induce cholestatic liver injury models.All the rats were monitored the bile flow of rats for 90 minutes,calculated the liver index of rats in each group.HE staining was used to observe the pathological changes of rat liver tissue.Transmission electron microscopy was used to observe the ultrastructure of liver cells.The automatic biochemical analyzer was used to detect alkaline phosphatase(ALP),alanine transaminase(ALT),aspartic acid transaminase(AST),γ-glutamyl transpeptidase(γ-GT),total bilirubin(TBil),direct bilirubin(DBil)and total bile acid(TBA)level of rat serum.Elisa method was used to detect the contents of superoxide dismutase(SOD),reduced glutathione(GSH)and malondialdehyde(MDA)in rat liver tissue.The expression of TNF-α,IL-1βand IL-6 mRNA in rat liver tissue were measured by qRT-PCR method.Results:Compared with the NOM group,the MOD group showed a decrease in bile flow at various time periods,while AST,ALT,ALP andγ-GT content increases,liver index increases,DBil,TBil and TBA levels increase,SOD activity and GSH,MDA levels decrease,TNF-α,IL-1βand IL-6 mRNA increased,and the differences were statistically significant(P<0.01).Compared with the ANIT group,the UDCA group had an increase in bile flow at 15,30,75,and 90 minutes,while the XHQD group had an increase in bile flow at all time periods.The UDCA and XHQD groups had AST,ALT,ALP andγ-GT content decreases,liver index decreases,DBil,TBil and TBA levels decrease,SOD activity and GSH,MDA levels increase,TNF-α,IL-1�
