摘 要:目的探讨中国朝鲜族2型糖尿病患者(T2DM)颈动脉硬化斑块(CAP)与线粒体ND4基因12026A→G突变的关系及其影响因素。方法回顾性选取2018年1月至2022年2月在延边大学附属医院内分泌科住院的744例朝鲜族T2DM患者作为研究对象,采用多态链式反应(PCR)技术对其进行线粒体DNA呼吸链复合物NADH脱氢酶第四亚单位(ND4)区域基因突变筛查。彩色多普勒超声(CDUS)观察患者CAP形成情况,筛选出CAP患者335例,无颈动脉硬化斑块(NCAP)患者409例。记录分析全部T2DM患者空腹血糖(FBG)、糖化血红蛋白(HbAlc)、C反应蛋白(CRP)、肌酐(Cr)、尿酸(UA)、总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白(LDL)。结果CAP与NCAP两组线粒体DNA 12026 A→G突变率比较,差异有统计学意义(P<0.05)。CAP组病程长于NCAP组,吸烟率、饮酒率、FBG、HbA1c、UA均高于NCAP组,差异有统计学意义(P<0.05)。logistic回归分析显示,吸烟(OR=0.49,95%CI:0.20~0.79)、FBG(OR=8.79,95%CI:2.45~38.66)和HbA1c(OR=9.01,95%CI:3.10~43.71)是CAP发生的独立危险因素(P<0.05)。结论线粒体DNA 12026 A→G突变与中国朝鲜族T2DM患者CAP形成有关。吸烟、FBG和HbA1c是CAP发生的独立危险因素。Objective To investigate the relationship between carotid atherosclerotic plaque(CAP)in Chinese Korean type 2 diabetes mellitus(T2DM)and mitochondrial ND4 gene 12026 A→G mutation and its influencing factors.Methods A total of 744 Korean T2DM patients admitted to the Department of Endocrinology,Yanbian University Affiliated Hospital from January 2018 to February 2022 were retrospectively selected as the study subjects.Polymorphic chain reaction(PCR)was used to detect mutations in the fourth subunit(ND4)region of mitochondrial DNA respiratory chain complex NADH dehydrogenase.CAP formation was observed by color Doppler ultrasound(CDUS).A total of 335 patients with CAP and 409 patients without carotid atherosclerotic plaque(NCAP)were screened.Fasting blood glucose(FBG),glycated hemoglobin(HbAlc),C-reactive protein(CRP),creatinine(Cr),uric acid(UA),total cholesterol(TC),triglyceride(TG),low density lipoprotein(LDL)of all patients with T2DM were recorded and analyzed.Results There was significant difference in the rate of mitochondrial DNA 12026 A→G mutation between CAP and NCAP groups(P<0.05).The course of disease in CAP group was longer than that in NCAP group,and the smoking rate,drinking rate,FBG,HbA1c and UA in CAP group were higher than those in NCAP group,and the differences were statistically significant(P<0.05).Logistic regression analysis showed that smoking(OR=0.49,95%CI:0.20-0.79),FBG(OR=8.79,95%CI:2.45-38.66)and HbA1c(OR=9.01,95%CI:3.10-43.71)were independent risk factors for CAP(P<0.05).Conclusion Mitochondrial DNA 12026 A→G mutation is associated with CAP formation in Chinese Korean T2DM patients.Smoking,FBG and HbA1c are independent risk factors for CAP.
