摘　　要：目的探讨持续被动关节活动(CPM)联合芒硝冷敷疗法对全膝关节置换术(TKA)后患者肿痛和功能恢复的影响。方法选取2022年1月至2023年4月于医院骨科接受TKA治疗的120例患者作为研究对象,根据术后干预方式不同分为CPM组、芒硝冷敷组和联合组,每组40例。CPM组给予连续2周的CPM训练,芒硝冷敷组给予连续7 d的芒硝+冰袋外敷,联合组采取以上两种干预方法。比较3组干预前后腿围、膝关节疼痛程度、术区皮肤温度和患肢活动度恢复正常时间。结果干预后第3、5天,芒硝冷敷组和联合组腿围均小于CPM组,差异有统计学意义(P<0.05)。干预后第3、5、7天,芒硝冷敷组和联合组的视觉模拟评分法(VAS)评分及患侧皮肤温度均低于CPM组,差异有统计学意义(P<0.05)。联合组的患肢活动度恢复正常时间短于CPM组和芒硝冷敷组,差异有统计学意义(P<0.05);CPM组患肢活动度恢复正常时间短于芒硝冷敷组,差异有统计学意义(P<0.05)。结论CPM联合芒硝冷敷应用于TKA后患者可发挥优势互补和协同作用,有助于消肿、止痛,降低局部温度,并加快患肢恢复正常活动功能。Objective To explore the effects of continuous passive motion(CPM)combined with mirabilite cold compress therapy on swelling,pain and functional recovery after total knee arthroplasty(TKA).Methods From January 2022 to April 2023,a total of 120 patients who underwent TKA in the hospital were recruited and randomly assigned into 3 groups according to the intervention methods,that is,CPM group,mirabilite cold compress group,and combined group with each group consisting of 40 cases.The CPM group received two weeks of GPM training,while the mirabilite cold compress group was treated with mirabilite combined with ice pack external application for 7 days.The combined group received the above two intervention methods.Before and after intervention,the leg circumference,pain of knee,skin temperature of surgical spot,and recovery time of affected limb activity of the affected limb were compared amony the 3 groups.Results On the 3rd and 5th days post-intervention,the leg circumference of the mirabilite cold compress group and the combined group were smaller than that of the CPM group,with statistically significant differences(P<0.05).On the 3rd,5th,and 7th days post-intervention,the visual analogue scale(VAS)scores and skin temperature of affected limb of the mirabilite cold compress group and the combined group were lower than those of the CPM group,with statistically significant differences(P<0.05).The recovery time of affected limb activity of the the combined group was shorter than those of the CPM group and mirabilite cold compress group,with statistically significant differences(P<0.05);The recovery time of affected limb activity of the CPM group was shorter than that of the mirabilite cold compress group,with a statistically significant difference(P<0.05).Conclusion The combined application of CPM training and mirabilite cold compress can produce complementary advantages and synergistic effects,thereby reducing swelling,providing analgesia,lowering local temperature,and shortening the recovery time of the affected lim
