扳机点针刺联合高频重复经颅磁刺激治疗带状疱疹后遗神经痛的临床观察

摘　　要：目的观察扳机点针刺联合高频重复经颅磁刺激治疗糖尿病带状疱疹后遗神经痛的临床效果。方法选取2015年12月—2019年12月在大连医科大学附属第二医院康复医学科确诊2型糖尿病合并带状疱疹后遗神经痛的45例患者,随机分为对照组和治疗组。对照组22例,给予传统针灸联合高频重复经颅磁刺激治疗;治疗组23例,给予扳机点针刺联合高频重复经颅磁刺激治疗。比较2组总有效率、治疗前后视觉模拟量表(VAS)评分、睡眠质量量表(SQS)评分、汉密尔顿焦虑量表(HAMA)评分,以及治疗前后空腹血糖数值。结果治疗组总有效率为91.30%(21/23),高于对照组的68.18%(15/22),差异有统计学意义(P<0.05)。在1个疗程、2个疗程后,2组VAS、HAMA评分均较前一时间点明显下降(P<0.01),SQS评分较前一时间点明显上升(P<0.01);且治疗组与同时间点的对照组比较,VAS、HAMA评分均明显下降,SQS评分明显上升,差异有统计学意义(P<0.05)。2组治疗前后空腹血糖数值比较,差异无统计学意义(P>0.05)。结论扳机点针刺联合高频重复经颅磁刺激治疗带状疱疹后遗神经痛效果显著,可改善患者的焦虑症状、提高患者睡眠质量,且对血糖影响小。Objective To observe the clinical effect of trigger point acupuncture combined with high frequency rTMS therapy on type 2 diabetes mellitus with postherpetic neuralgia.Methods Forty-five patients with type 2 diabetes complicated with postherpetic neuralgia diagnosed in The Second Affiliated Hospital of Dalian Medical University from December 2015 to December 2019 were selected and randomly divided into control group and treatment group.In the control group,22 cases were treated with traditional acupuncture combined with high frequency rTMS therapy.In the treatment group,23 cases were given trigger point acupuncture combined with high frequency rTMS therapy.The total effective rate,visual analog Scale(VAS)score,Sleep Quality Scale(SQS)score,Hamilton Anxiety Scale(HAMA)score,and fasting blood glucose before and after treatment were compared between the two groups.Results The total effective rate of the treatment group was 91.30%(21/23),which was higher than that of the control group(68.18%,15/22),and the difference was statistically significant(P<0.05).After one course and two courses,VAS and HAMA scores of the two groups were significantly decreased compared with the previous time point(P<0.01),and SQS scores were significantly increased compared with the previous time point(P<0.01).Compared with the control group at the same time point,VAS and HAMA scores were significantly decreased,while SQS scores were significantly increased,and the difference was statistically significant(P<0.05).There was no significant difference in fasting blood glucose between 2 groups before and after treatment(P>0.05).Conclusion The effect of trigger point acupuncture combined with high frequency rTMS therapy on postherpetic neuralgia is significant,which can improve anxiety symptoms and sleep quality of patients.It has little effect on blood sugar.
