夏黎明从阴阳论治食管癌临床经验总结

摘　　要：食管癌是一种常见的恶性肿瘤,是威胁全球公共卫生健康的重要因素。该病因为早期临床症状不易被发现,临床确诊时多伴随其他临床症状及转移等,给后续治疗带来较大难度,所以其综合治疗俨然成为急需解决的问题。中医药治疗肿瘤已有几千年的历史,中医古籍对于肿瘤的记载及治疗较为完善。夏黎明教授师承国医大师,拥有30多年的科研及临床经验,对于食管癌的综合治疗有较丰富的治疗方案,遂文章从病因病机、治疗方法、用药特点等几个方面对夏黎明教授“阴阳平衡”治疗食管癌的临床实践进行了经验总结。夏黎明基于阴阳平衡的理论,将食管癌的主要病因病机总结为两大方面:虚而为阴,包括气、血、阴、阳;实而为阳,包括痰、瘀、毒。阴阳失于平衡,日久伤于食管,发为噎膈之病。内生邪毒是食管癌发病的病理特点,应区分阴阳虚实,重视脾胃,疏肝补肾,重视放化疗及手术后的中药治疗。在疾病发展的过程中,阴阳可以互相转换,随着治疗的进行,毒邪会逐渐消失,从而达到补气温阳、驱邪滋阴的作用,以改善患者的生活质量,延长生存期。Esophageal cancer is a common malignant tumor and an important factor threatening the public health of people all over the world.Because the early clinical symptoms of esophageal cancer are not easy to be found,most patients are accompanied by other clinical symptoms and metastasis when clinically diagnosed,which brings great difficulties to the follow-up treatment,so its comprehensive treatment has become an urgent problem to be solved.TCM has been used to treat tumors for thousands of years,and the related TCM theory and treatment of tumors in ancient books are relatively perfect.Professor XIA Liming learned from TCM masters,has more than 30 years of scientific research and clinical experience,and has rich treatment plans for comprehensive treatment of esophageal cancer.Therefore,this article summarizes the clinical experience of Professor XIA Liming in treating esophageal cancer based on the theory of Yin(阴)and Yang(阳)balance from the aspects of etiology,pathogenesis,treatment method,medication characteristics,etc..Based on the theory of Yin and Yang balance,Professor XIA Liming summarizes the main causes and pathogenesis of esophageal cancer,which mainly includes two aspects:deficiency belongs to Yin,including Qi(气),blood,Yin and Yang,excess belongs to Yang,including phlegm,blood stasis and toxin.Yin and Yang are out of balance and hurt the esophagus for a long time,leading to hiccups.Endogenous toxin is the pathogenic characteristic of esophageal cancer.In the treatment of esophageal cancer,distinguishing Yin and Yang,and deficiency and excess should be emphasized to strengthen spleen and stomach,soothe liver and tonify kidney.Attention should also be paid to treatment with TCM medicine after radiotherapy,chemotherapy and surgery.During the evolution of the disease,Yin and Yang can be converted to each other.In the treatment of the disease,pathogenic factors will gradually disappear,so as to play the role of supplementing Qi and warming Yang,expelling pathogenic factors and nourishing Yin,in order to i
