摘　　要：术后肠麻痹是外科手术后由于非机械因素导致的胃肠功能恢复时间延长,致使患者无法经口摄入的疾病,属于中医学“肠痹”范畴。山东大学齐鲁医院杜广中主任从事手术后平滑肌动力障碍性疾病以及术后并发症的针灸临床研究20余年,临证经验丰富,对术后肠麻痹的诊治见解独到。杜广中主任认为,本病多以手术金刃损伤为主要病因,以通降失调、气机逆乱为主要病机。他提出“以通为主,重用中脘”“健运脾胃以固后天”“疏肝理气,以调气机”的治疗思路。临证时,他注重将西医体格检查与中医四诊合参相结合以评估患者病情,在针刺手法上以芒针刺、分刺为主,并配合使用电针。芒针刺中脘穴可直达病位,疏通中焦气机;分刺可增强得气感,更易激发经气;配以电针阳明经腧穴则加强补益气血、培补后天之效。文章结合案例介绍杜广中主任芒针治疗术后肠麻痹经验,以期为临床提供参考。Postoperative ileus is a disease caused by prolonged recovery time of gastrointestinal function due to non-mechanical factors after surgery,resulting in patients unable to ingest through the mouth,which belongs to the category of intestinal paralysis in TCM.DU Guangzhong,a director of Shandong University Qilu Hospital has been engaged in acupuncture and moxibustion clinical research on postoperative smooth muscle dynamic disorders and postoperative complications for more than 20 years,with rich experience in clinical diagnosis and treatment of postoperative ileus.Director DU believed that surgery injury was the main cause of the disease,and the disorder of the general health and the disturbance of the Qi(气)machine were the main pathogenesis.He put forward the treatment ideas of paying attention to Zhongwan point(CV12),strengthening the spleen and stomach and soothing the liver and regulating Qi.In clinical practice,he paid attention to the combination of physical examination of Western medicine and four diagnostic methods of TCM to assess the patient’s condition.In terms of acupuncture techniques,elongated needling and intermuscular needling were mainly used,and electroacupuncture was used in coordination.Acupuncture at Zhongwan point with elongated needle can directly reach the disease location and dredge the middle energizer.Intermuscular needling can enhance the sense of getting Qi and stimulating the meridian Qi more easily.The combination of electroacupuncture at Yangming meridian(阳明经)acupoints can replenish Qi and blood,and nourish the body.This article introduces DU Guangzhong’s experience in treating postoperative ileus with elongated needle by case analysis,hoping to provide a reference for clinical practice.
