快速按摩水道穴预防产后癃闭效果的Meta分析

摘　　要：目的:系统评价快速按摩水道穴预防产后癃闭的应用效果。方法:计算机检索PubMed、Web of Science、Embase等数据库中有关产后尿潴留文献,检索截止日期为2022年6月。由2名研究人员独立进行文献筛选、数据提取及文献质量评价,采用RevMan 5.3软件对符合要求的文献进行结果分析。结果:共纳入文献8篇,涉及干预组2492例、对照组2572例。Meta分析结果显示:快速按摩水道穴能够缩短产妇产后首次排尿时间[SMD=-1.5,95%CI(-1.88,-1.13),P<0.001];减少尿潴留的发生率[OR=0.13,95%CI(0.08,0.22),P<0.001];促进产妇产后首次排尿量[MD=74.34,95%CI(57.46,91.22),P<0.001];减少膀胱残余尿量[MD=-32.59,95%CI(-52.79,-12.4),P<0.001]及尿不尽的发生率[OR=0.23,95%CI(0.12,0.42),P<0.001],与对照组的差异具有统计学意义。结论:快速按摩水道穴预防产后癃闭具有一定的效果。但所纳入的文献有局限性,仍期待后续高质量试验加以补充。Objective:To systematically evaluate the effect of rapid massage at Shuidao point(ST28)in the prevention of postpartum urinary retention.Methods:The literature on postpartum urinary retention in PubMed,Web of Science,Embase and other databases was searched by computer.The deadline for searching was June 2022.Two researchers independently performed literature screening,data extraction and literature quality evaluation.RevMan 5.3 software was used to analyze the results of the documents that met the requirements.Results:A total of 8 papers were included,including 2492 cases in the intervention group and 2572 cases in the control group.The results of Meta-analysis showed that:Rapid massage at Shuidao points could shorten the time of first urination after delivery[SMD=-1.5,95%CI(-1.88,-1.13),P<0.001]and reduce the incidence of urinary retention[OR=0.13,95%CI(0.08,0.22),P<0.001],increased initial postpartum urine volume[MD=74.34,95%CI(57.46,91.22),P<0.001],decreased residual urine volume of bladder[MD=-32.59,95%CI(-52.79,-12.4),P<0.001]and the incidence of urinary insufficiency[OR=0.23,95%CI(0.12,0.42),P<0.001].The difference between the groups was statistically significant.Conclusion:Rapid massage at Shuidao point can prevent postpartum urinary retention in postpartum period,but the quality of the literature included in the analysis is limited,and more higher quality clinical randomized controlled trials are needed to verify the conclusion.
