四妙散加减治疗滑膜炎、痤疮、脓疱病、骨肥厚和骨髓炎综合征医案及探讨

摘　　要：滑膜炎、痤疮、脓疱病、骨肥厚、骨髓炎(Synovitis-acne-pustulosis-hyperostosis-osteomyelitis Syndrome,SAPHO综合征是以滑膜炎、痤疮、脓疱病、骨肥厚和骨炎症候群命名的自身免疫性疾病,亦是风湿免疫疾病中的罕见病。目前其病因和发病机制尚不明确,临床中极易被误诊误治。该病表现复杂,且容易复发,疼痛皮肤损伤极大地影响患者的生活质量。早发现、早控制骨破坏和皮肤病变,可以避免病理性骨折、降低感染的发生率,因此提早预防该病越来越受到中西医医学的重视。该病在治疗上仍面临挑战,目前西医尚没有统一的治疗指南,多采用对症治疗,生物制剂尚未普及。中医学对其病因病机、处方用药、个体调护等均有重要的论述,临床验案效果明显,具有借鉴价值及意义。文章结合案例介绍四妙散治疗SAPHO综合征经验,以期为临床提供参考。Synovitis-acne-pustulosis-hyperostosis-osteomyelitis(SAPHO)syndrome is an autoimmune disease named after synovitis,acne,impetigo,bone hypertrophy and bone inflammatory syndrome,and is also a rare disease in rheumatic immune diseases.At present,the etiology and pathogenesis of SAPHO syndrome are not clear,and it is easy to be misdiagnosed and mistreated in clinic.The disease has complex manifestations and is prone to recurrence.Pain and skin damage greatly affect the quality of life and mental health of patients.Early detection and control of bone destruction and skin lesions can avoid the incidence of pathological,so it has been paid more and more attention by TCM and Western medicine.The disease is still facing challenges in Western medicine.There is no unified treatment guideline,symptomatic treatment is the main response measure,and biological agents have not been popularized.TCM has an important discussion on its etiology and pathogenesis,prescription medication,individual care,etc..The clinical effect is obvious,which has a reference value and significance.This article introduces the experience of Simiao Powder in the treatment of SAPHO syndrome by case study in order to provide a reference for clinical practice.
