基于《当代名老中医典型医案集》探讨胸痹病的用药规律

摘　　要：目的:通过数据挖掘技术探讨当代名老中医治疗胸痹的用药规律,为临床用药提供指导。方法:收集《当代名老中医典型医案集》中关于胸痹的方剂,采用Microsoft Excel建立数据库,对单味中药及其性味归经进行频次分析;采用SPSS Modeler 18.0的Apriori模块对药物进行关联规则分析。结果:(1)共收集方剂53首,包括中药138味,药物累计频数698次,其中用药频次最多的是丹参,共出现37次。(2)用药四气五味包括寒、热、温、凉,平,总频次138次,其中温性药使用频次最多(56次,占药性总频次的40.6%),寒性药次之(40次,占总频数的29.0%);药味以甘味为主,占药味总频次的31.3%;药物多归肝经,占药物归经总频次的18.1%。(3)对高频药物进行关联规则分析,显示“置信度排名居前三位的为丹参-黄芪-川芎、甘草-茯苓-半夏、五味子-麦冬-丹参。支持度排名居前三位的为瓜蒌-薤白、丹参-黄芪、五味子-麦冬”。结论:初步探索出当代名老中医治疗胸痹仍以活血、滋阴、祛痰为主。Objective:To explore the medication rules of contemporary famous and old Chinese medicine in the treatment of chest pain,and provides guidance for clinical medication through data mining technology.Methods:The prescriptions for chest pain in Typical Medical Cases of Contemporary Famous Old Chinese Medicine Doctor were collected,and Microsoft Excel was used to establish a database to analyze the frequency of single TCM medicine and its nature and taste.The Apriori module of SPSS Modeler 18.0 was used to analyze the association rules of drugs.Results:①A total of 53 prescriptions were collected,including 138 TCM medicines,with a cumulative frequency of 698 times.Among them,Danshen(Radix et Rhizoma salviae Miltiorrhizae)has the highest use frequency of 37 times.The four Qi(气)include cold,hot,warm,cool and moderate,with a total frequency of 138 times,of which the use frequency of warm drugs is the highest(56 times,40.6%),followed by cold drugs(40 times,29.0%).The main flavor is sweet,accounting for 31.3%of the total frequency.Most of the drugs belong to the liver channel,accounting for 18.1%of the total frequency of the drugs.3.The association rule analysis of high-frequency drugs showed that“the top three of the confidence level were Danshen-Huangqi(Radix Astragali)-Chuanxiong(Rhizoma Chuanxiong),Gancao(Radix Glycyrrhizae)-Fuling(Poria)-Banxia(Rhizoma Pinelliae),Wuweizi(Fructus Schisandrae Chinensis)-Maidong(Radix Ophiopogonis)-Danshen.The top three of the support level were Gualou(Fructus Trichosanthis)-Xiebai(Bulbus Allii Macrostemonis),Danshen-Huangqi,Wuweizi-Maidang.Conclusion:It is preliminarily explored that the treatment of chest pain by contemporary famous TCM medicine doctors still focus on promoting blood circulation,nourishing Yin(阴)and eliminating phlegm.
