摘　　要：儿童髋关节错缝,又称为儿童髋关节一过性滑膜炎或者暂时性髋关节滑膜炎,是引起儿童跛行及髋部疼痛最常见的疾病之一,多为单侧发病,双侧发病少见。钟黎娟教授以中医理论为基础,认为本病的病因病机特点系本虚标实,儿童稚阴稚阳,感受外邪,气血瘀滞,痹阻于髋部而发病。钟教授根据儿童中医特性、临床实例,在原有的国家标准分型的基础上,将儿童髋关节错缝证型扩展为六型,即风寒阻络型、风热蕴结型、湿热痹阻型、肝火流筋型、血瘀气滞型及脾胃虚弱型。并对每一证型典型的临床症状体征以及舌脉象进行归纳总结,同时为每一证型列举了推荐用药基础方。以此六型中医辨证分型为基础,遣方用药,辨证施治,一方面大大提高了对儿童髋关节错缝的中医临床精准治疗,另一方面有力地提升了临床医师的中医诊治水平。本文总结钟黎娟教授治疗儿童髋关节错缝的临床经验,从儿童髋关节错缝的病因病机、中医辨证分型、处方用药等方面论述钟黎娟教授治疗本病的经验,并辅以临床治疗典型案例加以佐证,进一步充实了儿童髋关节错缝的中医证型,取得了良好的临床效果。Hip joint dislocation in children,also known as transient synovitis of hip joint or transient synovitis of hip joint in children,is one of the most common diseases causing claudication and hip pain in children.It is mostly unilateral and bilateral.Based on the theory of TCM,Professor ZHONG Lijuan believes that the etiology and pathogenesis of this disease are characterized by deficiency in origin and excess in superficiality.Children are immature in Yin(阴)and Yang(阳),feel external pathogens,Qi(气)and blood stasis,and blockage in the hip.According to the characteristics of children’s TCM medicine and clinical examples,Professor ZHONG extended the syndrome type of children’s hip joint dislocation into six types on the basis of the original national standard classification,namely:wind-cold obstruction type,wind-heat accumulation type,damp-heat obstruction type,liver-fire flowing tendon type,blood stasis and Qi stagnation type and spleen-stomach weakness type.The typical clinical symptoms and signs and tongue and pulse conditions of each syndrome type were summarized,and the basic prescriptions of recommended medication were listed for each syndrome type.Based on the six types of TCM syndrome differentiation,prescription medication,syndrome differentiation,on the one hand,greatly improve the clinical precise treatment of TCM medicine for children with hip dislocation,on the other hand,effectively improve the level of diagnosis and treatment of TCM medicine clinicians.Conclusion:Professor ZHONG summed up experience based on clinical examples,further enriched the TCM syndrome types of hip joint dislocation in children,and achieved good clinical results.This paper discusses professor ZHONG Lijuan’s experience in the treatment of children’s hip joint dislocation from the aspects of etiology and pathogenesis,TCM syndrome differentiation and typing,prescription and medication,supplemented by typical cases of clinical treatment.
