早期康复治疗在大面积烧伤患者中的临床应用及疗效观察

摘　　要：目的观察早期康复治疗在大面积烧伤患者中的临床应用效果。方法选取2020年1月至2022年12月我院收治的120例大面积烧伤[总体表面积(TBSA)≥50%]患者,采用随机数字表法分为早期组和常规组各60例,两组均给予常规治疗及创面愈合后功能锻炼,早期组在此基础上进行早期康复干预。比较两组股四头肌肌力、肘关节和腕关节主动活动度、心理状态评分、生活质量评分。结果干预6个月后,两组股四头肌伸膝峰力矩(PT)、总做功量(TW)、平均功率(AP)均高于干预前,早期组股四头肌肌力指标高于常规组(P<0.05);干预1、3、6个月后,两组肘关节和腕关节主动活动度高于干预前,且早期组肘关节和腕关节主动活动度高于常规组(P<0.05);干预6个月后,两组抑郁自评量表(SDS)评分、焦虑自评量表(SAS)评分均低于干预前,且早期组心理状态评分低于常规组(P<0.05);两组躯体、心理、社会功能及一般健康状况评分均高于干预前,且早期组生活质量评分高于常规组(P<0.05)。结论早期康复治疗有利于最大限度恢复大面积烧伤患者关节功能,改善患者心理状况,提高患者肌力和生活质量。Objective To observe the clinical application effect of early rehabilitation therapy in patients with large area burn.Methods A total of 120 patients with large area burn(total surface area(TBSA)≥50%)admitted to our hospital from January 2020 to December 2022 were selected.The patients were divided into an early group and a conventional group according to whether early rehabilitation intervention was performed,60 in each group.Both groups were given conventional treatment and functional exercise after wound healing.The early group received early rehabilitation intervention while the conventional group did not receive early rehabilitation intervention.The differences of quadriceps muscle strength,elbow and wrist active motion,mental state score and quality of life score were compared between the two groups.Results After 6 months of intervention,the quadriceps femoris peak torque(PT),total work(TW),and average power(AP)of the two groups were higher than those before the intervention,and the quadriceps muscle strength index of the early group was higher than that of the routine group(P<0.05).After 1 month,3 and 6 months of intervention,active motion of elbow and wrist in both groups was higher than that before intervention,and the motion in the early group was higher than that in the conventional group(P<0.05).After 6 months of intervention,the scores of Self-rating Depression Scale(SDS)and Self-rating Anxiety Scale(SAS)in the two groups were lower than before intervention,and the scores in the early group were lower than those in the conventional group(P<0.05).The scores of physical,psychological,social function and general health status of the two groups were higher than those before intervention,and the scores of life quality of the early group were higher than those of the conventional group(P<0.05).Conclusions Early rehabilitation of patients with large area burns is beneficial to maximize the recovery of joint function.It improves the psychological status,the muscle strength and the quality of life of the pa
