国外社区药师参与慢病管理实践及对我国的启示

摘　　要：在全球慢病管理发展进程中,药师作为不可替代的角色参与其中。目前国外药师在社会药房、社区医院及居家环境中提供药物治疗管理、慢病预防及筛查、药物使用审查等药学服务,并取得了显著成效。我国在分级诊疗制度背景下,出台了相关政策以推动基层药学服务发展,但目前社区药学服务尚处于起步阶段,需要更多的经验借鉴。该文通过对美国、英国、澳大利亚、日本4个国家社区药师参与慢病管理的工作模式和服务内容、政策文件、药师资质、药学服务收费机制进行梳理总结,为我国社区药师实践以及社区药学发展提供参考。Pharmacists play an irreplaceable role in the development process of global chronic disease management.Foreign pharmacists provide pharmacy services such as medication therapy management,chronic disease prevention and screening,and medication use review in social pharmacies,community hospitals,and home environments,and have achieved significant results.In China's hierarchical medical system,relevant policies have been introduced to promote the development of grassroots pharmacy services.However,community pharmacy services are still at an early stage and need more experience to learn from.This article provides a summary of the working models and service content,policy documents,pharmacist qualifications,and pharmacy service payment mechanisms of community pharmacists in the United States,United Kingdom,Australia,and Japan,providing a reference for the practice of community pharmacists and the development of community pharmacy in China.
