空心加压螺钉内固定与股骨头置换术治疗中老年股骨颈骨折的临床疗效比较

摘　　要：目的比较空心加压螺钉内固定与股骨头置换术治疗中老年股骨颈骨折的临床疗效。方法选取2017年1月至2020年12月本院收治的80例中老年股骨颈骨折患者作为研究对象,根据入院时间先后分为对照组与观察组,每组40例。对照组采用空心加压螺钉内固定术治疗,观察组采用股骨头置换术治疗,比较两组视觉模拟评分法(VAS)评分、髋关节Harris评分、术后并发症发生情况及髋关节优良率。结果术前,两组VAS评分、髋关节Harris评分比较差异无统计学意义;术后1个月,两组VAS评分均低于术前,髋关节Harris评分均高于术前,且观察组VAS评分低于对照组,髋关节Harris评分高于对照组,差异有统计学意义(P<0.05)。观察组髋关节总优良率为97.50%,高于对照组的85.00%,差异有统计学意义(P<0.05)。观察组术后并发症发生率为7.50%,低于对照组的27.50%,差异有统计学意义(P<0.05)。结论与空心加压螺钉内固定术相比,股骨头置换术治疗中老年股骨颈骨折疗效显著,能减轻患者疼痛,改善其髋关节功能,降低术后并发症发生风险,值得临床推广应用。Objective To compare the clinical effects of hollow compression screw internal fixation and femoral head replacement in the treatment of middle-aged and elderly femoral neck fractures.Methods 80 middle-aged and elderly patients with femoral neck fracture admitted to our hospital from January 2017 to December 2020 were selected as the study subjects,and they were divided into the control group and the observation group according to the time of admission,with 40 cases in each group.The control group was treated with hollow compression screw internal fixation,and the observation group was treated with femoral head replacement,the visual analogue scale(VAS)score,hip joint Harris score,the total excellent and good rate of hip jointand postoperative complications were compared between the two groups.Results Before operation,there were no significant differences of VAS score and hip joint Harris score between the two groups;1 month after operation,the VAS score of the two groups were lower than before operation,and the hip joint Harris score were higher than before operation,and the VAS score in the observation group was lower than that in the control group,and the hip joint Harris score was higher than that in the control group,and the differences were statistically significant(P<0.05).The total excellent and good rate of hip joint in the observation group was 97.50%,which was higher than 85.00%in the control group,and the difference was statistically significant(P<0.05).The total incidence of postoperative complications in the observation group was 7.50%,which was lower than 27.50%in the control group,and the difference was statistically significant(P<0.05).Conclusion Compared with hollow compression screw internal fixation,femoral head replacement is effective in the treatment of middle-aged and elderly femoral neck fracture,which can relieve pain,improve hip joint function,and reduce the risk of postoperative complications,it is worthy of clinical promotion and application.
