摘 要:目的研究电子计算机断层扫描(computed tomography,CT)以及核磁共振成像(nuclear magnetic resonance imaging,MRI)诊断膝关节损伤的价值。方法选取2021年1月—2023年2月于南京市六合区人民医院放射科检查的164例疑似膝关节损伤患者为研究对象,经关节镜检测作为金标准确诊,其中膝关节损伤患者82例。对所有研究对象予以CT检测以及MRI检测,收集并分析不同检测方式的准确度、特异度、灵敏度以及不同膝关节损伤类型检出数据。结果MRI检测准确度为98.78%、特异度为98.78%、灵敏度为98.78%;CT检测准确度为88.41%、特异度为90.24%、灵敏度为86.59%;在82例膝关节损伤中检测出骨质改变9例,半月板损伤16例,内侧副韧带损伤10例,外侧副韧带损伤11例,前交叉韧带损伤12例,后交叉韧带损伤12例,关节腔积液12例。MRI检测对半月板损伤类型诊断符合率高于CT检测,差异有统计学意义(P<0.05)。两种检测方法其余类型诊断符合率比较,差异无统计学意义(P>0.05)。结论在针对膝关节损伤进行检测时,选择MRI检测能够提高诊断的准确性,能够针对不同膝关节损伤予以鉴别,建议在实际诊断中予以参考。Objective To investigate the value of computed tomography(CT)and nuclear magnetic resonance imaging(MRI)in the diagnosis of knee injuries.Methods During the period from January 2021 to February 2023,164 patients with suspected knee injuries were recorded in the Department of Radiology of Nanjing Liuhe District People's Hospital,and the diagnosis was confirmed by arthroscopic detection as the gold standard,among which 82 patients with knee injuries were recorded.CT and MRI tests were performed on all the study subjects,and the accuracy,specificity,sensitivity,and detection data of different testing modalities were collected and analyzed for different types of knee injuries.Results The accuracy of MRI was 98.78%,the specificity was 98.78%,and the sensitivity was 98.78%.CT detection accuracy was 88.41%,specificity was 90.24%,and sensitivity was 86.59%.Bone changes were detected in 9,meniscal injuries in 16,medial collateral ligament injuries in 10,lateral collateral ligament injuries in 11,anterior cruciate ligament injuries in 12,posterior cruciate ligament injuries in 12,and articular effusion in 12 of 82 knee injuries.The diagnostic compliance rate for the type of meniscus injury by MRI detection was higher than that by CT detection,and the difference was statistically significant(P<0.05).Compared the remaining types of diagnostic compliance rate between the two detection methods,the difference was not statistically significant(P>0.05).Conclusion In the detection of knee injuries,MRI can improve the diagnostic accuracy and identify different knee injuries,which is recommended to be referred to in the actual diagnosis.
