“太极云手”联合高流量湿化氧疗治疗吸入性肺炎并肺实变的临床研究

摘　　要：目的:探讨“太极云手”联合高流量湿化氧疗治疗吸入性肺炎并肺实变的临床价值。方法:将福建中医药大学附属康复医院于2021年3月至2022年9月收治的61例吸入性肺炎并肺实变患者按信封法分为对照组(n=30)和研究组(n=31)。所有患者到院后均接受常规内科治疗,对照组在此基础上增加高流量湿化氧疗治疗,研究组在对照组基础上增加“太极云手”治疗,两组均连续治疗2周。比较两组治疗效果、临床相关指标(抗生素使用时间、住院时间)肺部X线片分级、呼吸功能指标[补吸气量(IC)、补呼气量(ERV)、潮气量(VT)、肺活量(VC)、用力肺活量(FVC)]和炎症介质[白细胞介素-1(IL-1)、C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)]水平。结果:研究组治疗总有效率为100.00%,高于对照组的80.00%(P<0.05);研究组抗生素使用时间及住院时间短于对照组(P<0.05);治疗后,研究组肺部X线片分级优于对照组(P<0.05);治疗后,研究组IC、ERV、VT及FVC高于对照组(P<0.05);治疗后,研究组IL-1、CRP及TNF-α水平低于对照组(P<0.05)。结论:“太极云手”联合高流量湿化氧疗可有效促进吸入性肺炎并肺实变患者肺部病灶吸收,改善其呼吸功能,减轻机体炎症反应,促进患者康复。Objective:To explore the clinical value of"Tai Chi Cloud Hand"combined with high flow humidified oxygen therapy in the treatment of inhalation pneumonia with pulmonary consolidation.Method:61 patients with aspiration pneumonia and pulmonary consolidation admitted to the Rehabilitation Hospital Affiliated to Fujian University of Traditional Chinese Medicine from March 2021 to September 2022 were divided into a control group(n=30)and a study group(n=31)according to the envelope method.After arriving at the hospital,all patients received routine internal medicine treatment.The control group received high flow humidified oxygen therapy on top of this,while the study group received"Tai Chi Cloud Hand"treatment on top of the control group for 2 consecutive weeks.Compare the treatment efficacy,clinical related indicators(antibiotic use time,hospital stay),lung X-ray grading,respiratory function indicators[inspiratory volume(IC),expiratory volume(ERV),tidal volume(VT),vital capacity(VC),forced vital capacity(FVC)],and inflammatory mediators[interleukin-1(IL-1),C-reactive protein(CRP),tumor necrosis factor-α(TNF-α)]between two groups.Result:The total effective rate of the study group was 100.00%,which was higher than the 80.00%of the control group(P<0.05);The duration of antibiotic use and hospitalization in the study group was shorter than that in the control group(P<0.05);After treatment,the lung X-ray grading of the study group was better than that of the control group(P<0.05);After treatment,the IC,ERV,VT,and FVC in the study group were higher than those in the control group(P<0.05);After treatment,the study group showed IL-1,CRP,and TNF levels-αThe level was lower than that of the control group (P<0.05). Conclusion: The combination of "Tai Chi Cloud Hand" and high flow humidified oxygen therapy can effectively promote the absorption of lung lesions in patients with inhalation pneumonia and pulmonary consolidation, improve their respiratory function, reduce the inflammatory response of the body, and promote rapid r
