血清FGF2表达水平与急性呼吸窘迫综合征患者病情严重程度及预后的关系

摘　　要：目的 分析急性呼吸窘迫综合征(acute respiratory distress syndrome,ARDS)患者血清碱性成纤维细胞生长因子2(fibroblast growth factor 2,FGF2)表达水平与病情严重程度以及预后的关系。方法 收集2020年6月~2022年6月在我院接受治疗的ARDS患者126例作为研究对象,根据治疗后30天预后情况将其分为存活组(72例)与死亡组(54例),根据氧合指数[动脉血氧分压(arterial partial oxygen pressure,PaO_(2))/吸入氧浓度(inhaled oxygen concentration,FiO_(2))]分为轻度组(42例),中度组(48例),重度组(36例)。使用ELISA法检测各组血清FGF2水平;采用Spearman相关性分析ARDS患者血清FGF2的表达水平与急性生理学与慢性健康状况评分系统Ⅱ(APACHEⅡ)评分之间的关系;Logistic回归分析影响ARDS患者预后的因素;运用受试者工作特征(ROC)曲线分析血清FGF2水平对ARDS患者预后的预测价值。结果 与存活组相比,死亡组ARDS患者血清FGF2表达水平以及APACHEⅡ评分升高,PaO_(2)/FiO_(2)降低(P<0.05)。轻度组、中度组、重度组FGF2表达水平、APACHEⅡ评分依次升高(P<0.05)。Spearman相关分析结果显示,ARDS患者血清中FGF2水平与APACHEⅡ评分呈正相关(r=0.565,P<0.05)。Logistic回归分析结果显示,APACHEⅡ、FGF2、PaO_(2)/FiO_(2)是ARDS患者预后的影响因素(P<0.05)。ROC曲线结果发现,FGF2预测ARDS患者预后的曲线下面积(AUC)为0.913,最佳截断值分别为28.014 pg/mL,灵敏度和特异度分别为79.60%和91.70%。结论 ARDS患者血清FGF2水平与病情严重程度和预后密切相关,且FGF2对ARDS患者预后具有一定的预测价值。Objective To analyze the relationship between the expression level of serum basic fibroblast growth factor 2(FGF2)and the severity and prognosis of patients with acute respiratory distress syndrome(ARDS).Methods From June 2020 to June 2022,126 ARDS patients who were treated in our hospital were regarded as the research objects,according to the 30d prognosis after treatment,they were grouped into a survival group(72 cases)and a death group(54 cases),according to the oxygenation index(PaO_(2)/FiO_(2)),they were grouped into a mild group(42 cases),moderate group(48 cases),and severe group(36 cases).Serum FGF2 level in each group was detected by ELISA;Spearman correlation was applied to analyze the relationship between serum FGF2 expression level and Acute Physiology And Chronic Health Evaluation System II(APACHE II)score in ARDS patients;Logistic regression was applied to analyze the factors affecting the prognosis of ARDS patients;receiver operating characteristic(ROC)curve was applied to analyze the predictive value of serum FGF2 level on the prognosis of ARDS patients.Results Compared with the survival group,the serum FGF2 expression level and APACHE II score of the ARDS patients in the death group were increased,and the PaO_(2)/FiO_(2) was decreased(P<0.05).The expression level of FGF2 and APACHEⅡscore in mild,moderate,and severe groups was increased successively(P<0.05).The results of Spearman correlation analysis showed that serum FGF2 level was positively correlated with the APACHEⅡ score ( r =0.565, P <0.05). Logistic regression analysis showed that APACHE Ⅱ, FGF2, and PaO_(2)/FiO_(2) were prognostic factors of ARDS patients ( P <0.05). ROC curve results showed that the area under the curve (AUC) of FGF2 for predicting the prognosis of ARDS patients was 0.913, the optimal cut-off value was 28.014 pg/mL, and the sensitivity and specificity were 79.60% and 91.70, respectively.
