多层螺旋CT影像诊断腹内疝的临床价值

摘　　要：目的探讨腹内疝的多层螺旋CT(MSCT)影像学表现及临床价值。方法收集并分析12例本院经手术证实为腹内疝的急诊MSCT检查的资料,所有患者在常规非增强全腹部MSCT容积扫描后作多平面重组处理。结果12例患者均有肠梗阻征象,其中先天性腹内疝4例,为十二指肠左旁疝3例,右旁疝1例;继发性腹内疝8例,均为粘连索带疝;12例中9例在多平面重组(MPR)上可见疝口,3例未见明确疝口,疝口处肠管、肠系膜及血管走行聚集呈“鸟嘴征”和“缆绳征”;12例中4例有完整疝囊结构,5例为扩张肠管壁构成的假性疝囊,3例未见疝囊;12例疝入的肠管呈C型/反C型、“假肾征”改变。结论MSCT容积扫描结合MPR重组诊断腹内疝引起的肠梗阻具有重要的临床价值。Objective To investigate the imaging findings and clinical value of multiple-slice spiral CT(MSCT)in internal hernia.Methods The data of emergency MSCT examination of 12 patients with internal abdominal hernia confirmed by operation were collected and analyzed.All patients underwent multiplanar reconstruction after routine non-enhanced total abdominal MSCT volume scan.Results All 12 patients had signs of intestinal obstruction,including 4 cases with a congenital internal hernia,3 cases with a left para-duodenal hernia,1 case with a right para-duodenal hernia,8 cases with secondary abdominal internal hernia,all of which were adhesive band hernia.Hernia opening was found in 9 cases on multiplanar reformation(MPR),and no clear hernia opening was found in 3 cases.The intestinal tube,mesentery and blood vessels at the hernia opening were clustered with beak sign and cable sign.Among the 12 cases,4 cases had complete hernia sac structure,5 cases were pseudo hernia sac formed by dilated intestinal wall,3 cases had no hernia sac,and the herniated intestine showed type C/anti-C type and pseudorenal sign in 12 cases.Conclusion MSCT volume scan combined with MPR reconstruction has important clinical value in the diagnosis of intestinal obstruction caused by abdominal internal hernia.
