保留喉功能手术在老年局部晚期下咽癌患者中的应用效果观察

摘　　要：目的了解保留喉功能手术在老年局部晚期下咽癌患者中的应用价值。方法70例老年局部晚期下咽癌患者按手术方式的不同分为对照组和观察组,每组35例。对照组采用传统全喉切除术,观察组采用喉保留手术。对比两组患者治疗3个月后的生活质量、并发症发生情况及2 a无复发生存率(RFS)、总生存率(OS)。结果治疗3个月后观察组患者的生理机能、生理职能、躯体疼痛、一般健康状况、精力、社会功能、情感职能和精神健康评分均明显高于对照组(P<0.05或0.01)。两组并发症发生情况及2 a RFS、OS比较差异无统计学意义(P>0.05)。结论保留喉功能手术可提高老年局部晚期下咽癌患者的生活质量,但不影响2 a RFS及OS,也不增加并发症发生率。Objective To investigate the application value of laryngeal preservation surgery in elderly patients with locally advanced hypopharyngeal carcinoma.Methods A total of 70 elderly patients with locally advanced hypopharyngeal carcinoma were divided into the Control Group and the Observation Group according to the surgical method,35 cases in each group.The Control Group was treated with traditional total laryngectomy,while the Observation Group was treated with laryngeal preservation surgery.The quality of life,the incidence of complications,2 a relapse free survival(RFS),and 2 a overall survival(OS)were compared between the two groups.Results At 3 months after treatment,the scores of physiological functions,body pain,general health status,energy,social function,emotional function and mental health in the Observation Group were significantly higher than those in the Control Group(P<0.05 or 0.01).There were no significant differences in the incidence of complications as well as 2 a RFS and 2 a OS between the two groups(P>0.05).Conclusion Laryngeal preservation surgery can improve the quality of life of elderly patients with locally advanced hypopharyngeal cancer,without affecting the 2 a RFS and 2 a OS,nor increasing the incidence of complications.
