摘 要:目的探讨呼气末二氧化碳分压(Pet CO_(2))监测技术在儿科急诊中的应用价值。方法选取74例紧急气管插管患儿,记录使用Pet CO_(2)判定气管插管位置的时间;在插管后0和0.5 h记录Pet CO_(2)值及动脉血二氧化碳分压(Pa CO_(2)),分析Pet CO_(2)与Pa CO_(2)相关性。结果采用Pet CO_(2)判定气管插管位置所需的平均时间为13.5 s。插管后0和0.5 h,Pet CO_(2)与Pa CO_(2)呈正相关(r值分别为0.997、0.994,P<0.01)。Bland-Altman法显示Pet CO_(2)与Pa CO_(2)具有良好一致性。结论使用Pet CO_(2)判定气管插管位置准确、快捷;Pet CO_(2)与Pa CO_(2)呈正相关,可通过无创Pet CO_(2)监测实时反映患儿体内Pa CO_(2)情况。Objective To observe the application of end-tidal carbon dioxide (Pet CO_(2)) monitoring in pediatric emergency.Methods The time of Pet CO_(2) to judge the position of endotracheal intubation was recorded in 74 children undergoing emergency endotracheal intubation.Pet CO_(2) and partial arterial carbon dioxide pressure (Pa CO_(2)) were measured at 0 h and 0.5 h after intubation.The correlation between Pet CO_(2) and Pa CO_(2) was analyzed.Results The average time of Pet CO_(2) to determine the position of endotracheal intubation was 13.5 seconds.Pet CO_(2) was positively correlated with Pa CO_(2) at 0 h (r=0.997,P<0.01)and 0.5 h (r=0.994,P<0.01) after intubation.Bland-Altman method showed a good consistency between Pet CO_(2) and Pa CO_(2).Conclusion Pet CO_(2) is accurate and rapid to determine the position of tracheal intubation.The noninvasive Pet CO_(2) monitoring can real-timely reflect Pa CO_(2) in children due to their positive correlation.
