摘 要:目的:探讨不同股骨固定方式对前交叉韧带(ACL)解剖重建效果的影响。方法:回顾性选取2019年2月—2021年12月于贺州市中医医院行前交叉韧带重建术(ACLR)的70例患者作为研究对象,其中采用Endobutton带袢钢板固定方式的38例患者为Endobutton组,采用Rigidfix横穿钉固定方式的32例患者为Rigidfix组。比较两组患者术后膝关节功能、膝关节稳定性、隧道扩大情况和并发症发生情况。结果:与术前相比,两组术后1年的Lysholm评分、国际膝关节文献委员会(IKDC)评分、Tenger评分均明显升高(P<0.05),但术后1年组间Lysholm评分、IKDC评分、Tenger评分及前抽屉试验(ADT)、Lachman试验、轴移试验和KT-1000侧-侧差值对比,差异均无统计学意义(P>0.05);Endobutton组D1值显著大于Rigidfix组(P<0.05),D2值、D3值、D4值与Rigidfix组比较,差异均无统计学意义(P>0.05);Endobutton组并发症发生率为7.89%,虽低于Rigidfix组的21.88%,但差异无统计学意义(P>0.05)。结论:Endobutton带袢钢板与Rigidfix横穿钉固定方式应用于ACLR,均可改善膝关节功能,维护其稳定性,安全可靠,但Endobutton固定方式更易引起骨隧道扩大。Objective:To investigate the effect of different femoral fixation methods on anatomical reconstruction of anterior cruciate ligament(ACL).Method:Seventy patients who underwent ACL reconstruction(ACLR)in Hezhou Hospital of Traditional Chinese Medicine from February 2019 to December 2021 were selected as the study subjects.Among them,38 patients undergoing Endobutton loop steel plate fixation were defined as the Endobutton group,and 32 patients undergoing Rigidfix cross-pins fixation were defined as the Rigidfix group.Postoperative knee function,knee joint stability,tunnel expansion and complications were compared between the groups.Result:Lysholm scores,IKDC scores and Tenger scores were significantly higher in both groups 1 year after surgery than those before surgery(P<0.05),however,there were no statistical significance in Lysholm scores,IKDC scores,Tenger scores,anterior drawer test(ADT),Lachman test,axis shift test and KT-1000 side to side difference among groups 1 year after surgery(P>0.05).The D1 value of Endobutton group was significantly higher than that of Rigidfix group(P<0.05),and there were no statistical significance in D2 values,D3 values and D4 values compared with Rigidfix group(P>0.05).The incidence of complications in Endobutton group was 7.89%,lower than 21.88%in Rigidfix group,but the difference was not statistically significant(P>0.05).Conclusion:The application of Endobutton loop steel plate fixation and Rigidfix cross-pins fixation in ACLR both can improve knee function and maintain its stability,both methods are safe and reliable,but Endobutton loop steel plate fixation is more likely to cause bone tunnel expansion.
