慢性鼻窦炎经鼻内镜鼻窦手术后局部应用乙酰半胱氨酸的效果观察

摘　　要：目的:观察慢性鼻窦炎(CS)经鼻内镜鼻窦手术后局部应用乙酰半胱氨酸的疗效。方法:选取2018年6月—2019年10月深圳市中西医结合医院收治的98例CS患者,利用随机数字表法分为对照组与研究组,每组49例。对照组经鼻内镜鼻窦手术后给予常规治疗,研究组在对照组基础上局部应用乙酰半胱氨酸。比较两组治疗效果、疼痛程度、炎症因子水平、Lund-Kennedy内镜评分、鼻黏液纤毛输送率(MTR)及生活质量。结果:研究组总有效率明显高于对照组(P<0.05);研究组治疗后视觉模拟评分法(VAS)评分明显低于对照组(P<0.05);研究组治疗后白细胞介素-1β(IL-1β)、白细胞介素-5(IL-5)、白细胞介素-6(IL-6)、白细胞介素-17(IL-17)水平均明显低于对照组(P<0.05);研究组治疗后MTR明显快于对照组(P<0.05),Lund-Kennedy内镜评分和鼻腔鼻窦结局测试-20(SNOT-20)总分均明显低于对照组(P<0.05)。结论:CS经鼻内镜鼻窦手术后局部应用乙酰半胱氨酸的临床效果良好,能够有效降低术后疼痛感,降低炎症因子水平和改善鼻功能,有助于提高患者术后的生活质量。Objective:To observe the curative effect of local application of Acetylcysteine after transnasal endoscopic sinus surgery for chronic sinusitis(CS).Method:A total of 98 patients with CS treated in Shenzhen Hospital of Integrated Chinese and Western Medicine from June 2018 to October 2019 were selected and divided into control group and study group according to random number table method,with 49 cases in each group.The control group received conventional treatment after transnasal endoscopic sinus surgery,and the study group applied Acetylcysteine locally on the basis of the control group.The treatment effect,pain degree,inflammatory factors level,Lund-Kennedy endoscopic score,mucociliary transport rate(MTR)and quality of life were compared between the two groups.Result:The total effective rate of the study group was significantly higher than that of the control group(P<0.05).After treatment,the visual analogue scale(VAS)score of the study group was significantly lower than that of the control group(P<0.05).After treatment,the levels of interleukin-1β(IL-1β),interleukin-5(IL-5),interleukin-6(IL-6)and interleukin-17(IL-17)in the study group were significantly lower than those in the control group(P<0.05).After treatment,MTR of the study group was significantly faster than that of the control group(P<0.05),Lund-Kennedy endoscopic score and sino-nasal outcome test-20(SNOT-20)score were significantly lower than those of the control group(P<0.05).Conclusion:The local application of Acetylcysteine after transnasal endoscopic sinus surgery for CS has good clinical effect,which can effectively reduce the levels of inflammatory factors,improve nasal function,and contribute to improving the postoperative quality of life of patients.
