加速康复外科护理结合肢体训练对关节镜下肩袖修复后肩关节功能和神经功能的影响

摘　　要：目的:探究加速康复外科(enhanced recovery after surgery,ERAS)护理结合肢体训练对关节镜下肩袖修复(arthroscopic rotator cuff repair,ARCR)后肩关节功能和神经功能的影响。方法:选取2021年1月—2022年8月肩关节损伤至中国人民解放军联勤保障部队第九〇九医院骨科收治的行ARCR手术的100例患者,将其随机分为两组,即研究组和对照组,各50例。对照组在整个研究中给予常规护理,研究组接受ERAS护理和肢体训练。对比两组视觉模拟评分法(visual analogue scale,VAS)评分,体侧外旋角度、主前屈角度、美国肩肘外科协会(American shoulder and elbow surgeons,ASES)评分、美国加州大学(University of California at Los Angeles,UCLA)肩关节评分系统评分、美国国立卫生研究院卒中量表(national institute of health stroke scale,NIHSS)评分及并发症发生率。结果:干预前,两组各项指标对比,差异均无统计学意义(P>0.05)。研究组在术后第1、4、8、12周的VAS评分均明显低于对照组(P<0.05)。干预3个月后,研究组体侧外旋角度、主动前屈角度、ASES、UCLA肩关节评分系统、NIHSS评分均明显优于对照组,并发症发生率低于对照组(P<0.05)。结论:ERAS护理结合肢体训练能明显缓解患者术后疼痛,加快肩关节功能的恢复,改善神经损伤,可以用于关节镜下肩袖损伤修复后患者功能的恢复中。Objective:To explore the effect of enhanced recovery after surgery(ERAS)nursing combined with limb training on shoulder joint function and nerve function after arthroscopic rotator cuff repair(ARCR).Method:A total of 100 patients due to shoulder joint injury admitted to the 909th Hospital of Chinese People's Liberation Army Joint Logistic Support Force undergoing ARCR operation from January 2021 to August 2022 were randomly divided into two groups,the study group and the control group,with 50 cases in each group.The control group accepted conventional nursing during the whole study,the study group received ERAS nursing and limbs training.The visual analogue scale(VAS),lateral external rotation angle,active forward bending angle,American shoulder and elbow surgeons(ASES)scores,University of California at Los Angeles(UCLA)shoulder joint scoring system scores,national institute of health stroke scale(NIHSS)scores and incidence of complications were compared.Result:Before intervention,there were no statistically significant differences various indicators between the two groups(P>0.05).The VAS scores in the study group were significantly lower than those in the control group at postoperative 1 week,4 weeks,8 weeks,12 weeks(P<0.05).After 3 months of intervention,the lateral lateral rotation angle,active flexion angle,ASES,UCLA shoulder joint scoring system,NIHSS scores in the study group were apparently better than those in the control group,and the incidence of complications was lower than that in the control group(P<0.05).Conclusion:ERAS nursing combined with limb training can significantly relieve patients'postoperative pain,speed up the recovery of shoulder joint function,improve nerve injury,and can be used for the recovery of patients'function after the repair of shoulder sleeve injury under arthroscopy.
