摘　　要：目的:探究自体脂肪移植联合序贯激光治疗烧伤后增生性瘢痕(HS)的效果。方法:选取贵溪市中医院2021年1月—2022年2月收治的82例烧伤后HS患者,按照随机数字表法分为观察组(41例)与对照组(41例),对照组接受序贯激光治疗,观察组接受自体脂肪移植联合序贯激光治疗,比较两组临床疗效、瘢痕恢复情况[改良温哥华瘢痕量表(mVSS)]、瘢痕厚度、瘢痕血流灌注量及对治疗效果的满意率。结果:与对照组相比,观察组治疗总有效率更高,差异有统计学意义(P<0.05);治疗前,两组mVSS评分、瘢痕厚度、瘢痕血流灌注量差异均无统计学意义(P>0.05),治疗后,观察组mVSS评分、瘢痕厚度与血流灌注量均低于对照组,差异均有统计学意义(P<0.05);观察组对治疗效果的满意率高于对照组,差异有统计学意义(P<0.05)。结论:予以烧伤后HS患者采用自体脂肪移植联合序贯激光治疗可获得较好的效果,有助于瘢痕的消退,改善瘢痕厚度及血流灌注情况,患者对治疗的满意度较高。Objective:To explore the effect of autologous fat transplantation combined with sequential laser in the treatment of hypertrophic scar(HS)after burn.Method:A total of 82 patients with post-burn HS admitted to Guixi Traditional Chinese Medicine Hospital from January 2021 to February 2022 were selected and divided into observation group(41 cases)and control group(41 cases)according to the random number table method.The control group received sequential laser therapy,while the observation group received autologous fat transplantation combined with sequential laser therapy.The clinical efficacy,scar recovery[modified Vancouver scar scale(mVSS)],scar thickness scar blood perfusion,and satisfaction rate with treatment effect were compared between the two groups.Result:Compared with the control group,the total effective rate of the observation group was higher,the difference was statistically significant(P<0.05).Before treatment,there were no significant differences in mVSS score,scar thickness and scar blood perfusion between the two groups(P>0.05),after treatment,the mVSS score,scar thickness and scar blood perfusion volume in the observation group were lower than those in the control group,the differences were statistically significant(P<0.05).The satisfaction rate of the observation group was higher than that of the control group,the difference was statistically significant(P<0.05).Conclusion:Autologous fat transplantation combined with sequential laser therapy for patients with HS after burns can achieve good effects,which is helpful to the regression of scar,improve scar thickness and blood perfusion,and patients have high satisfaction with the treatment.
