复方黄柏液涂剂联合VSD对Wagner 2、3级糖尿病足患者炎症因子及足部血供情况的影响

摘　　要：目的:探讨复方黄柏液涂剂联合负压封闭引流(VSD)对Wagner 2、3级糖尿病足患者炎症因子及足部血供情况的影响。方法:选取2021年10月—2022年10月在九江市第一人民医院接受治疗的糖尿病足患者82例,按随机数字表法分为两组。VSD组(n=41)给予VSD治疗,联合组则在VSD组的基础上加用复方黄柏液涂剂治疗。对比两组治疗效果、足部创面面积、炎症指标及足部血供情况。结果:联合组的总有效率(95.12%)高于VSD组(78.05%)(P<0.05);治疗2、4、6周后,联合组足部创面面积均小于VSD组(P<0.05);两组治疗后的C反应蛋白(CRP)、CD64、白介素-6(IL-6)水平均低于治疗前,且联合组均低于VSD组(P<0.05);两组踝肱指数(ABI)、经皮氧分压(TcPO_(2))、趾肱指数(TBI)均高于治疗前,且联合组均高于VSD组(P<0.05)。结论:与单一使用VSD治疗Wagner 2、3级糖尿病足相比,联合复方黄柏液涂剂可有效提高治疗效果,减轻机体炎症反应,改善足部血供情况,促进创面愈合。Objective:To investigate the effects of Fufang Huangbaiye Liniment combined with vacuum sealing drainage(VSD)on inflammatory factors and foot blood supply of patients with Wagner grade 2,3 diabetic foot.Method:A total of 82 diabetic foot patients who received treatment in Jiujiang NO.1 People's Hospital from October 2021 to October 2022 were selected and divided into two groups according to random number table method.The VSD group(n=41)was treated with VSD,and the combination group was treated with Fufang Huangbaiye Liniment on the basis of VSD group.The therapeutic effect,foot wound area,inflammation index and foot blood supply were compared between the two groups.Result:The total effective rate of combination group(95.12%)was higher than that of VSD group(78.05%)(P<0.05).After 2,4 and 6 weeks of treatment,the foot wound area of the combination group were smaller than those of the VSD group(P<0.05).After treatment,the levels of C reactive protein(CRP),CD64 and interleukin-6(IL-6)in both groups were lower than those before treatment,and those in combination group were lower than those in VSD group(P<0.05).Ankle brachial index(ABI),transcutaneous partial pressure of oxygen(TcPO_(2))and toe brachial index(TBI)in both groups were higher than those before treatment,and those in combination group were higher than those in VSD group(P<0.05).Conclusion:Compared with the single use of VSD in the treatment of Wagner grade 2,3 diabetic feet,combined with Fufang Huangbaiye Liniment can effectively improve the therapeutic effect,reduce the inflammatory response of the body,improve the foot blood supply and promote wound healing.
