热敏灸及中药颗粒熏鼻联合西医治疗儿童慢性鼻-鼻窦炎的临床研究

摘　　要：目的:探究热敏灸及中药颗粒熏鼻联合西医治疗儿童慢性鼻-鼻窦炎的临床效果。方法:选取2021年3月—2022年9月赣州市人民医院收治的86例慢性鼻-鼻窦炎患儿,按随机数字表法分为两组,各43例。对照组采取西医治疗,试验组在西医治疗基础上加用热敏灸及中药颗粒熏鼻治疗。比较两组临床疗效、视觉模拟评分法(VAS)评分、改良Lund-Kennedy(MLK)鼻内镜评分系统、不良反应。结果:治疗结束时,试验组总有效率较对照组高(P<0.05)。治疗后2、4、12周,两组鼻塞、流涕、咳嗽、头痛VAS评分较治疗前均降低,且随着时间延长各症状VAS评分呈降低趋势,且试验组均较对照组低(P<0.05)。治疗后2、4、12周,两组MLK评分较治疗前均降低,且随着时间延长呈逐渐降低趋势,且试验组均较对照组低(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:在西医治疗儿童慢性鼻-鼻窦炎基础上采用热敏灸及中药颗粒熏鼻可以提高疗效,减轻患儿鼻塞、流涕等临床症状,且不增加不良反应。Objective:To explore the clinical effect of heat sensitive moxibustion and traditional Chinese medicine Granules for nasal fumigation combined with western medicine in the treatment of chronic rhinosinusitis in children.Method:A total of 86 children with chronic rhinosinusitis admitted to Ganzhou People's Hospital from March 2021 to September 2022 were selected,and divided into two groups by random number table method,with 43 cases in each group.The control group was treated with western medicine,while the experimental group was treated with heat sensitive moxibustion and traditional Chinese medicine granules for nasal fumigation on the basis of western medicine.The clinical efficacy,visual analogue scale(VAS)score,modified Lund-Kennedy(MLK)nasal endoscopic scoring system scores and adverse reactions between two groups were compared.Result:At the end of treatment,the total effective rate of the experimental group was higher than that of the control group(P<0.05).After 2 weeks,4 weeks,and 12 weeks of treatment,the VAS scores of nasal congestion,runny nose,cough and headache in both groups decreased compared to before treatment,and the VAS scores of each symptom showed a decreasing trend over time,and those of the experimental group were lower than those of the control group(P<0.05).After 2 weeks,4 weeks,and 12 weeks of treatment,MLK scores of both groups decreased compared to before treatment,and showed a gradual decreasing trend over time,and those of the experimental group were lower than those of the control group(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:On the basis of treating chronic sinusitis in children with western medicine,using heat sensitive moxibustion and traditional Chinese medicine granules for nasal fumigation can improve the therapeutic effect,alleviate clinical symptoms such as nasal congestion and runny nose in children,and do not increase adverse reactions.
