大柴胡汤联合西药对肥胖型2型糖尿病患者胰岛素抵抗的影响

摘　　要：目的分析大柴胡汤联合西药对肥胖型2型糖尿病(T2DM)患者胰岛素抵抗(IR)的影响。方法选取2019年7月至2021年7月收治的130例肥胖型T2DM患者作为研究对象,按照随机分配原则将其分为对照组(n=65,西药治疗)和研究组(n=65,西药治疗基础上给予大柴胡汤治疗)。比较两组的治疗效果。结果研究组的治疗总有效率显著高于对照组(P<0.05)。治疗后,研究组的超氧化物歧化酶(SOD)、过氧化氢酶(CAT)水平高于对照组,丙二醛(MDA)、活性氧簇(ROS)水平低于对照组(P<0.05)。治疗后,研究组的空腹胰岛素水平、稳态模型评估-胰岛素抵抗指数(HOMA-IR)低于对照组,胰岛β-细胞功能指数(HOMA-β)高于对照组(P<0.05)。治疗后,研究组的腹腔内脂肪面积(VFA)、腹内脂肪体积(IAFV)、脂肪细胞脂肪酸结合蛋白(A-FABP)水平低于对照组(P<0.05)。结论大柴胡汤联合西药治疗肥胖型T2DM可从氧化应激、脂肪面积方面调节患者的IR,效果显著。Objective To analyze the effect of Dachaihu decoction combined with western medicine on insulin resistance(IR)in patients with obese type 2 diabetes mellitus(T2DM).Methods A total of 130 obese patients with T2DM from July 2019 to July 2021 were selected as the research objects,and the patients were divided into control group(n=65,western medicine treatment)and study group(n=65,Dachaihu decoction treatment on the basis of western medicine treatment)according to the principle of random distribution.The therapeutic effects of the two groups were compared.Results The total effective rate of treatment in the study group was significantly higher than that in the control group(P<0.05).After treatment,the levels of super oxide dismutase(SOD)and catalase(CAT)in the study group were higher than those in the control group,and the levels of malondialdehyde(MDA)and reactive oxygen species(ROS)were lower than those in the control group(P<0.05).After treatment,the fasting insulin level and homeostasis model assessment of insulin resistance(HOMA-IR)in the study group were lower than those in the control group,and the homeostasis model assessment-β(HOMA-β)was higher than that in the control group(P<0.05).After treatment,the visceral fat area(VFA),intra-abdominal fat volume(IAFV)and adipocyte fatty acid binding protein(A-FABP)levels in the study group were lower than those in the control group(P<0.05).Conclusion Dachaihu decoction combined with western medicine in the treatment of obese T2DM can regulate the IR of patients from the aspects of oxidative stress and fat area,and has remarkable effect.
