食管胃底静脉曲张出血经内镜下组织胶联合套扎治疗后早期再出血的影响因素

摘　　要：目的:分析内镜下组织胶联合静脉曲张套扎治疗食管胃底静脉曲张破裂出血患者早期再出血及影响因素.方法:选取2020年2月至2022年9月本院收治的79例食管胃底静脉曲张破裂出血患者为研究对象,分析内镜下组织胶联合静脉曲张套扎治疗后患者的早期再出血情况及影响因素.结果:79例患者未出血者69例(87.34%),出血者10例(12.66%);出血组与未出血组的肝功能(Chid-Pugh)分级、红细胞比容(HCT)、血小板计数(PLT)和血红蛋白(HGB)表达比较,差异有统计学意义(P<0.05).Chid-Pugh分级、HCT、PLT、HGB水平是影响食管胃底静脉曲张破裂出血患者治疗后早期再出血的独立危险因素(P<0.05).结论:食管胃底静脉曲张破裂出血患者采用内镜下组织胶联合静脉曲张套扎治疗后仍有一定再出血率,对于合并危险因素者,临床需做好预防方案,降低再出血发生率.Objective:To analyze the early rebleeding of patients with esophageal and gastric fundus variceal hemorrhage treated by endoscopic tissue glue combined with variceal ligation and its influencing factors.Methods:A total of 79 patients with esophageal and gastric varices rupture and hemorrhage admitted to hospital from February 2020 to September 2022 were collected.Analyze the incidence rate of early rebleeding of patients after endoscopic tissue gum combined with varicose vein ligation treatment and its influencing factor.Results:Among the 79 patients,69 cases(87.34%)had no bleeding and 10 cases(12.66%)had bleeding.There were significant differences in Chid-Pugh grading,hematocrit(HCT),platelet count(PLT)and hemoglobin(HGB)expression between the bleeding group and the non-bleeding group(P<0.05).Chid-Pugh grade,HCT,PLT and HGB levels were independent risk factors for early rebleeding in patients with esophageal and gastric variceal hemorrhage after treatment(P<0.05).Conclusion:There is still a certain rate of rebleeding in patients with esophageal and gastric fundus variceal rupture and bleeding after the treatment of endoscopic tissue glue combined with variceal ligation.For patients with combined risk factors,clinical prevention programs should be made to reduce the incidence of rebleeding.
