摘 要:目的:探讨外周血瘦素(leptin)、摄食抑制因子(nesfatin-1)、脂肪因子趋化素(chemerin)对妊娠期糖尿病(GDM)母婴结局的预测作用.方法:选取2018 年1 月至2020 年3 月龙岩市第一医院收治154 例GDM 患者作为研究组、另选同期131 例健康孕妇作为对照组.两组行外周血leptin、nesfatin-1、chemerin 水平检测.分析这三种物质与GDM 母婴不良结局的关系.结果:研究组血清leptin、nesfatin-1、chemerin 水平均高于对照组,差异有统计学意义(P<0.05);外周血leptin、nesfatin-1、chemerin 水平均是GDM 母婴不良结局的危险因素,三者联合预测GDM 母婴不良结局的曲线下面积与灵敏度高于各指标单独预测,差异有统计学意义(P<0.05).结论:血清leptin、nesfatin-1、chemerin 联合检测对GDM 患者母婴不良结局具有良好的预测价值.Objective:To explore the predictive effect of Leptin,Nesfatin-1 and Chemerin in peripheral blood on maternal and neonatal outcomes of gestational diabetes mellitus(GDM).Methods:One hundred and fifty-four patients GDM from January 2018 to March 2020 admitted to Longyan first hospital were selected as the research group,and 131 healthy pregnant women were selected as the control group,and the levels of Leptin,Nesfatin-1 and Chemerin in peripheral blood of them were detected.The relationships between Leptin,Nesfatin-1,Chemerin and maternal and infant adverse outcomes in GDM were analyzed.Results:The levels of serum Leptin,Nesfatin-1 and Chemerin in the study group were higher than those in the control group(P<0.05).The levels of peripheral blood Leptin,Nesfatin-1 and Chemerin were the risk factors of adverse maternal and neonatal outcomes of GDM,and the area under the curve and sensitivity of the combined prediction of these three for adverse maternal and neonatal outcomes in GDM were higher than those of each index alone(P<0.05).Conclusion:The combined detection of the serum Leptin,Nesfatin-1 and Chemerin has a good predictive value for adverse maternal and neonatal outcomes of patients with GDM.
