摘　　要：肝移植是治疗终末期肝病最有效的方法,术后免疫抑制方案与受者的长期存活和生存质量密切相关。哺乳动物雷帕霉素靶蛋白抑制剂(mammalian target of rapamycin inhibitor,mTORi)是一种三烯大环内酯类免疫抑制剂,具有抑制T细胞增殖、诱导受者免疫耐受、减轻钙调磷酸酶抑制药(calcineurin inhibitor,CNI)的肾脏毒性和有效抑制肿瘤细胞生长等作用。近年肝移植受者使用以m TORi为基础的免疫抑制方案逐渐增多。为了更好地为国内肝移植临床医师提供参考,中国器官移植发展基金会组织国内多中心移植专家,针对m TORi在肝移植中的作用机制和临床应用、推荐剂量和血药浓度监测、不良反应及应对措施等4个方面,经广泛征求意见,共同制订本专家共识。Liver transplantation is considered the most effective treatment for end-stage liver disease.The success of the procedure and the quality of life for recipients are closely linked to the postoperative immunosuppressive regimens.One such regimen is the use of mammalian target of rapamycin inhibitor(mTORi),a type of immunosuppressive agent that inhibits T cell proliferation,promotes immune tolerance,reduces renal toxicity caused by calcineurin inhibitors(CNI),and effectively inhibits tumor cell growth.In recent years,there has been a gradual increase in the use of mTORi-based immunosuppressive regimens by liver transplant recipients.To provide valuable guidance for liver transplant clinicians in China,the China Organ Transplantation Development Foundation has collaborated with experts from multiple transplant centers to develop this expert consensus.The consensus covers four key areas:the mechanism of action and clinical application of mTORi in liver transplantation,recommended dosage and monitoring of blood drug concentration,adverse reactions and treatment.Through extensive consultation,this expert consensus has been formulated.
