摘 要:目的:分析男性骨质疏松症患者肠道菌群结构特点并分析骨密度与血清学指标之间的关系。方法:于2023年3月-5月收集符合纳入与排除标准的12例男性骨质疏松患者与同时期体检的8例骨量正常者的粪便和血液样本,进行细菌16S rRNA测序和血清学指标检测,分析其与骨密度的相关性。结果:不同骨密度患者在肠道菌群属水平存在统计学差异(P<0.01),男性骨质疏松患者肠道菌群α多样性要低于骨量正常组,罗氏菌属(Roseburia)、琼脂杆菌(Agaribacter)、巨球型菌(Megasphaera)和另枝杆菌(Alistipes)等菌属丰度下降,物种功能分析显示其与环境信息加工、细胞过程、人类疾病、新陈代谢等功能相关。骨密度与血尿酸呈负相关(r=-0.5995,P=0.0052),与血清TGF-β1呈正相关(r=0.4692,P=0.0369),二者可用来诊断男性骨质疏松症(均P<0.01)。结论:男性骨质疏松患者的肠道菌群多样性下降,血尿酸、TGF-β1水平与骨密度相关,可用于骨质疏松症的诊断。Objective To analyze the structural characteristics of gut microbiota in male patients with osteoporosis and the relationship between bone density and serological indicators.Methods From March to May 2023,fecal and blood samples were collected from 12 male osteoporosis patients who met the inclusion and exclusion criteria,as well as 8 normal bone mass individuals who underwent concurrent physical examinations.Bacterial 16S rRNA sequencing and serological indicators were performed to analyze their correlation with bone density.Results There were statistical differences in the level of gut microbiota among patients with different bone densities(P<0.01),and the alpha diversity of gut microbiota in male patients with osteoporosis was lower than that of the control group with normal bone density,with a decrease in the abundance of genera,such as Roseburia,Agathobacter,Megasphaera and Alistipes.Species functional analysis shows that they were related to environmental information processing,cellular processes,human diseases,metabolism and other functions.Bone density was negatively correlated with blood uric acid(r=-0.5995,P=0.0052)and positively correlated with serum TGF-β1(r=0.4692,P=0.0369),and both can be used to diagnose male osteoporosis(both P<0.01).Conclusion Decreased gut microbiota diversity and the levels of blood uric acid,and TGF-β1in male patients with osteoporosis are related to bone density,which can be used for the diagnosis of osteoporosis.
