局部麻醉在下肢浅静脉曲张腔内射频消融术中的临床效果

摘　　要：目的探讨局部麻醉在下肢浅静脉曲张腔内射频消融术(RFA)中的临床效果。方法收集2020年12月至2021年12月绍兴文理学院附属医院收治的122例下肢浅静脉曲张RFA患者的临床资料,按照不同麻醉方式将其分为对照组(n=61,采用椎管内麻醉)与观察组(n=61,采用局部麻醉)。比较两组患者的手术指标、疼痛情况、满意度、并发症发生情况。结果观察组患者的手术时间、术后首次下床时间、住院时间均明显短于对照组患者,差异均有统计学意义(P﹤0.01)。术中,观察组患者视觉模拟评分法(VAS)评分高于对照组患者,差异有统计学意义(P﹤0.05);术后,两组患者视觉模拟评分法(VAS)评分比较,差异无统计学意义(P﹥0.05)。观察组患者总满意度为98.4%(60/61),高于对照组患者的85.2%(52/61),差异有统计学意义(P﹤0.05)。观察组患者头痛、腰痛及排尿障碍麻醉相关并发症发生率均低于对照组患者,差异均有统计学意义(P﹤0.05)。结论局部麻醉下RFA治疗下肢浅静脉曲张安全可行,患者恢复快,并发症少,满意度高,值得在临床上应用。Objective To investigate the clinical effect of local anesthesia in intracavitary radiofrequency ablation(RFA)for superficial varicose veins of lower extremity.Method The clinical data of 122 RFA patients with superficial varicose veins of the lower extremity admitted to the Affiliated Hospital of Shaoxing University from December 2020 to December 2021 were collected.They were included in the control group(n=61,using spinal anesthesia)and the observation group(n=61,using local anesthesia)according to different anesthesia methods.The surgical indicators,pain conditions,satisfaction,and occurrence of complications were compared between the two groups of patients.Result The operation time,first time out of bed after surgery,and hospitalization time of the patients in the observation group were significantly shorter than those of the patients in the control group,the differences were statistically significant(P<0.05).The intraoperative visual analogue scale(VAS)score of the observation group was higher than that of the control group,the difference was statistically significant(P<0.05);there was no statistically significant difference in VAS scores between the two groups(P>0.05).The total satisfaction of patients in the observation group was 98.4%(60/61),higher than 85.2%(52/61)of patients in the control group,the difference was statistically significant(P<0.05).The incidence rate of anesthesia related complications such as headache,low back pain,and urination disorders in the observation group weres lower than those of the control group,the differences were statistically significant(P<0.05).Conclusion RFA under local anesthesia for the treatment of lower extremity superficial varicose veins is safe and feasible,with fast patient recovery,fewer complications,and high satisfaction.It is worthy of clinical application.
